In addition to clinical responsibilities, the PLC-RN serves as a central coordinator, facilitating communication and collaboration among referring facilities, physicians, nursing staff, and various hospital departments. They work closely with hospital leadership and transfer center teams to streamline patient transitions, reduce delays, and maintain continuity of care.
