Registered Nurse II

Summary

The section of Gastro is seeking a competent and reliable Registered Nurse II to support our clinicians providing services to patients. This position will assist in the care of patients under the direction of our team of physicians. The RN II position is expected to demonstrate clinical competence when providing care, using technology, administering medications, performing procedures, managing emergencies, and acting as a patient and family advocate in order to monitor and maintain patient rights. The Registered Nurse is responsible for assisting in the delivery of health care and patient care management.

The ideal candidate must possess a proactive, team-oriented attitude. Position should be able to perform nursing and administrative duties using the principles of team-based and patient-centered care, chronic care management models, and quality improvement. It is the responsibility of this position to contribute to a patient centric and customer focused environment where the needs of the patients come first.

Job Duties

• Performs responsibilities in a manner that promotes quality and safe patient care and a positive patient experience.
• Maintains awareness of clinic activity, supports teamwork, and responds positively to requests for assistance.
• Provides and modifies care for complex patients using clinical judgment and evidence-based approaches.
• Adheres to the scope of practice as defined by the Texas Nurse Practice Act.
• Provides professional nursing assessment of patients.
• May ensure efficient operation of the clinic or unit by scheduling and planning for use of staff, supplies, and equipment.
• Monitors clinic workflow and delegates tasks to meet patient, clinic, and provider needs.
• Assists with treatment room examination and procedures on patients.
• Coordinates patient visits, facilitates referrals, and schedules follow-up appointments.
• Documents patient care services by charting in patient’s electronic medical record.
• Accurately documents all patient interactions, procedures, and education in EMR.
• Instructs patients concerning the preparations needed for tests and surgical procedures.
• Provides patient education on self-care, medication use, nebulizers/inhalers, diabetic care, wound care, and self-injections.
• Collaborates with providers, staff, and administrators to foster healthy relationships and ensure continuity of care.
• Executes physician orders and administers medications and treatments for patients.
• Administers vaccines, medications, and treatments as ordered.
• Offers ongoing education and counseling to patients and families to support independence and adherence.
• Provides telephone triage to patients and ensures patients receive the right level of care.
• Responds to patient messages, triages calls, and provides advice for minor acute problems, referring to PCP/APN/PA as needed.
• Assists with onboarding and training of new team members.
• Respects and supports patient rights and promotes positive family and patient relations.
• Maintains professionalism and patient-centered care in all interactions.
• Prioritizes and organizes work so that assignments are completed in an efficient and timely manner.
• Delegates tasks and monitors workflow to ensure efficiency.
• Identifies and supports basic cost containment and efficiency for patients and the clinic.
• Participates in quality improvement activities and supports resource management.
• Maintains clean and safe work environment; stocks, and orders treatment room supplies.
• Collaborates across departments and participates in team-based initiatives.
• Engages in additional tasks as needed to support clinic operations and patient care.
• Performs other job-related duties as assigned.
  
  

Minimum Qualifications

• Associate's Degree or Bachelor's Degree in Nursing.
• Two years of relevant experience as a licensed Registered Nurse (RN).
• Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.
• Basic Life Support (BLS) Certification.
