The Nurse-Family Partnership (NFP) Nurse establishes a trusting, therapeutic one-on-one relationship with low-income, first-time mothers who voluntarily consent to home visits throughout pregnancy and until the child reaches two years of age. The nurse conducts comprehensive assessments of the family’s physical, emotional, social, and environmental needs and provides counseling, health education, referrals, and case management related to prenatal and postpartum care, nutrition, parenting, child health and development, family planning, and life-course development.
The nurse-client partnership is the foundation for change, as the nurse supports clients in clarifying personal goals for themselves and their children and developing the skills needed to achieve those goals.
Responsibilities
- Conduct home visits in accordance with NFP Model Elements and Visit-to-Visit Guidelines
- Apply the nursing process to implement the NFP model in practice
- Provide education, support, and referrals to assist women and families in achieving targeted goals
- Collaborate and consult with interdisciplinary professionals involved in family services
- Formulate nursing diagnoses based on assessments and client-identified goals
- Plan and sequence home visits considering client acuity, geographic location, and program priorities
- Evaluate client progress toward stated goals and NFP-defined outcomes
Requirements
- Baccalaureate Degree in Nursing
- Licensed and currently registered to practice as a Registered Nurse in New York State
- Minimum of two (2) years of recent experience in maternal-child health
- Public/community health or home visiting experience preferred
- Current CPR certification required
- Excellent written and verbal communication skills
- Basic computer proficiency
- Valid driver’s license and insured automobile required
- Ability to work a flexible schedule, including some evenings and weekends, per agency requirements
