POSITION SUMMARY:

The Director of Nursing Services' primary responsibility is to ensure the provision of quality nursing care on a 24-hour basis to the residents of the Masonic Homes in accordance with Federal, State and Local standards and regulations.

The Director of Nursing assumes authority, responsibility, and accountability for the delivery of nursing services in the facility. In collaboration with facility Administration, allocates department resources in an efficient and economic manner to enable each resident to attain or maintain the highest practical physical, mental, and psychosocial well-being. Collaborates with other departments, medical professionals, consultants, and organizations, including government agencies and advocacy groups, to develop, support and coordinate resident care, related administrative functions, and to represent the interests of the facility.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

A. ADMINISTRATIVE FUNCTIONS:

  • Oversees administrative functions in Nursing Department.
  • Develops, maintains, and implements nursing policies and procedures that conform to current standards of nursing practice, facility philosophy, and operational policies while maintaining compliance with state and federal laws and regulations.
  • Communicates and interprets policies and procedures to nursing staff, and monitors staff practices and implementation.
  • Continuously monitors Federal and State guidelines and requirements and revises Departmental procedures accordingly.
  • Makes routine inspections of the facility to assure that established policies and procedures are being implemented and followed.
  • Responsible for the recruitment and hiring of both licensed and non-licensed personnel.
  • Functions as a liaison between the residents, their families, staff and the general public - responding to inquiries and providing information on nursing care and services.
  • Assists with the preparation of the Departmental annual budget and maintains the operating budgets for personnel and medical equipment and makes budget recommendations as to the purchase of such equipment.  Assures nursing staff properly charges out ancillaries used.
  • Helps the Administrator prepare staff for inspection surveys, instructing staff on matters of conduct and disclosure, being interviewed by inspectors, immediate corrections of problems noted by surveyors, etc.
  • Develops and maintains documentation systems for continuity of care and record storage that assures compliance with local, state and federal regulations and Masonic Homes policies and procedures.
  • Perform administrative requirements such as completing forms, writing reports, etc., and submit to the Administrator or appropriate agency as directed.
  • Recommends and collaborates in in-service education programs to promote staff development.
  • Understands and incorporates the departmental goals and objectives into daily actions.
  • Develop and maintain a good working-relationship with Federal, State and local agencies and with the physicians.
  • Develop systems of coordination among the nursing staff.
  • Review facility services to insure their implementation.
  • Evaluates the work performance of all nursing personnel, assists in the determination of wage increases, and implements discipline according to operational policies.
  • Collaborates with physicians, consultants, community agencies, and institutions to improve the quality of services and to resolve identified problems.
  • Oversees nursing schedules to assure they meet resident needs and regulatory and budgetary standards.
  • Oversees and supervises development and delivery of in-service education to equip nursing staff with sufficient knowledge and skills to provide compassionate, quality care and respect for resident rights.
  • Proactively develops positive employee relations, incentives, and recognition programs. Promotes teamwork, mutual respect, and effective communication.
  • Attend Department Head Meetings.
  • Acts in an administrative capacity in the absence of the Administrator.

B. CLINICAL FUNCTIONS:

  • Ensures delivery of compassionate quality care and nursing supervision as evidenced by adequate services and staff coverage on unit, absence of odors, general cleanliness, prevention of pressure wounds, and apparent maintenance of optimal resident functions.
  • Reviews 24-hour report from every unit daily to monitor and ensure timely, effective responses to significant changes in condition, transfers, and discharges, use of restraints, unexplained injuries, falls, behavioral episodes, and medication errors.
  • Coordinates nursing services with all other departments including Therapy.
  • Exercises overall supervision of resident assessments and care plans.
  • Participates in daily or weekly management team meetings to discuss resident status, census changes, personnel, or resident complaints or concerns.
  • Monitors the success of the Restorative Care Program in providing restorative nursing that maximizes each resident's existing abilities. Collaborates with the contract therapy company to enhance the quality of care for the residents.
  • Monitors the quality and type of care provided on all units, and the resident outcomes influenced by restorative care programming.
  • Performs rounds to observe care and to interview staff, residents, families or other interested parties.
  • Screen prospective residents to assure that staff has the ability to provide the care needed. Participates in all admission decisions, and may visit prospective residents before admission.
  • Assist in emergency situations involving the residents.
  • Monitors staff for compliance with OSHA mandates and facility policies on workplace safety. Proactively develops procedures and incentives to promote workplace safety and safe work practices.
  • Promotes customer service and hospitality and responds to and adequately resolves complaints or concerns from residents or families about nursing services.
  • Monitors facility incidents and complaints daily to identify those defined as unusual occurrences by State policy and promptly reports such occurrences to Administrator for appropriate action.
  • Monitors complaint reports daily for allegations of potential abuse or neglect, or the loss or misappropriation of resident property, and initiates  the investigation process.
  • Promotes compliance with accident prevention procedures, safety rules, and safe work practices to prevent employee injury and illness and control worker’s compensation costs.
  • Interacts courteously with residents, family members, employees, visitors, vendors, business associates, and representatives of government agencies.

C. QUALITY IMPROVEMENT

  • Demonstrates knowledge of and application of Key Clinical Quality Indicators, and proactively monitors and implements systems to achieve and/or surpass company thresholds.
  • Attend the Quality Assessment and Assurance Committee monthly to report on all data collection results from monthly audits and the selected plans of correction.  Plans of correction will be written by the Department involved or in conjunction with Quality Assurance and reported to the Committee.
  • Coordinates and/or develops on-going QA activities for nursing services to monitor nursing compliance with standards and regulatory requirements through rounds, interviews, and record reviews. Compiles summary of findings for the Quality Assurance committee.
  • Prepares Plan of Correction response to an inspection survey, and implements any follow-up QA required for any nursing allegations.
  • Assist in the coordination of studies performed within the Quality Assurance program and prepare reports as required.
  • Monitors the infection control program designed to provide a safe, sanitary, and comfortable environment designed to prevent the development and transmission of disease and infection.
  • Develop and implement appropriate plans of action to correct identified deficiencies as needed.
  • Integrates the MDS process with the quality indicators.
  • Make written and oral reports/recommendations to the Administrator, as necessary/required, concerning the operation of the Quality Assessment and Assurance program.
  • Ensures all nursing personnel are performing their duties according to the acceptable nursing standards.
  • Conducts quality assurance projects as they relate to survey readiness;.
  • Advises the facility administrator, regarding potential resident care or life problems;
  • Assist in reeducation of staff when developing corrective action plans from completed audits or chart reviews.  Insure documentation of remediation and report to Department Head.
  • Appoints quality improvement teams as needed for problem solving.
  • Facilitates discussion at QA&A committee to assure satisfaction with action taken and outcomes.
  • Prepares a written summary of accomplishments for the year.
  • Prepares a Quality Management work plan for the coming year.

D.RESIDENT CARE

  • Plans, develops and implements procedures and policies that enhance the quality of life for people receiving services and assures procedures that support people managing their own health care.
  • Provide resident-directed services that honor individual lifestyles and preferences as outlined in their plan of care.
  • Monitor staff approaches to care and services to assure that all residents have an opportunity to direct their own care and services, balancing clinical and psychosocial needs, and are treated fairly and with kindness, dignity, and respect.
  • Make resident visits as needed to observe and evaluate resident's physical and emotional status and revise care plan accordingly.
  • Support staff in creating an environment in their neighborhood where residents have ongoing opportunities to participate in individualized and purposeful activities, resembling their previous home environment, lifestyle and customary daily routine
  • Acts in a professional manner that places resident care above all else.
  • Ensure confidentiality of all resident care information.
  • Ensure that all nursing and personal care is provided in privacy.
  • Ensure all nursing service personnel are knowledgeable of residents’ rights and responsibilities.
  • Ensure that each resident’s care is resident-directed and enables him/her to reach the highest practical level of functioning as outlined in their plan of care

KNOWLEDGE, SKILLS AND ABILITIES:

  • Ability to perform nursing care per current community standard for long term care.
  • Demonstrates ability to lead and mentor staff.  Act as a resource person for all departments during shift within the established facility guidelines.
  • Interact effectively with residents, visitors, administration, physicians, and staff;
  • Knowledge of State and Federal laws and regulations governing Long Term Care.
  • Ability to read, write and speak English in a clear manner.
  • Experience with Microsoft Office applications including email; ability to learn company software systems.
  • Remain current on training and instructional methodologies, including applicable technology resources available.

Educational Requirements/Licenses/Certificate

  • Five years of professional nursing experience in the long-term care setting is required, and a minimum of three years’ experience in nursing administration or a comparable management position.  Previous DON experience in a SNF setting preferred.
  • Must possess and demonstrate a high degree of leadership, organizational ability, and communication skills.
  • Knowledge of the RAI/PPS process required; RAC-CT preferred.
  • Knowledge of long-term care management, policies, budgeting, personnel management, and census development is required.
  • Current CPR certification.
  • Graduate from an accredited nursing program. A BSN is required. 
  • Currently licensed as RN to practice in the State of California

As Primary Nurse, coordinates the activities of health care team members in completing diagnostic and therapeutic interventions. 4 Evaluates the effectiveness of nursing care delivered to reach the expected outcome. a) Evaluates the plan of care according to the current condition and changing needs of the patient. b) Accurately and concisely documents the patient’s status and response to the care delivered. Clinical Practice: Emergency Nursing 6 Utilizing professional nursing knowledge and skills. Appropriately assesses for: Physical and psychosocial-emotional needs. Barriers to learning. Educational/learning needs. Discharge needs. Age-related needs. Issues related to assault or abuse. 7 Analyzes patient related data, such as vital signs, past medical history, physical exam findings, laboratory values and other information, including age-specific criteria, to determine appropriate care. 8 Utilizes advanced, specialized knowledge and skills to implement interventions for all Emergency Services patients, by prioritizing needs, and using a patient centered approach. 9 Provides health education and teaching to patient’s, family members and/or significant others, that is age specific, providing for any barriers to learning. Clearly communicates health, treatment and discharge information, ensuring the patient, family member or significant other understands all discharge and follow-up instructions. 10 Effectively performs procedures, such as initiation of IV catheters and fluids, medication administration, dressings, patient teaching, etc., within their scope of practice, and unit, department and Medical Center policies and procedures. 11 Effectively performs other nursing procedures/skills, such as cardiac monitoring, rhythm interpretation, defibrillation, monitoring of conscious sedation, ABGs etc., based completed specialty training and their scope of practice, and unit, department and Medical Center policies and procedures. 12 Appropriately recognizes and quickly responds to emergency medical situations. a) Accurately assesses “A-B-Cs.” b) Initiates appropriate emergency treatment, i.e., CPR, O2, etc. c) Communicates critical information to other team members. 13 Maintains all required credentials and training (CA RN license, BLS, ACLS, PALS, NIHSS, AB508). 14 Remains current on emergency treatments/practices/procedures through attendance at staff meetings, continuing education events, in-services and the review of professional literature. 15 Integrates/applies new knowledge from education and research into clinical practice. 16 Demonstrates knowledge of and competence in age-related patient care. 17 Consistently demonstrates knowledge of state and federal regulations, such as Title XXII and COBRA/EMTALA. Follow all TJC, CDPH, CMS guidelines. 18 Advocates for public health and safety through patient and family education. 19 Consistently completes required documentation of the Emergency Services visit. 20 Consistently demonstrates caring and compassion through behaviors and interactions in performance of all duties, recognizing the inherent worth and dignity of patients, families, other staff and physicians. 21 Will perform duties of Relief Charge RN if called upon to do so. 22 Adheres to departmental and medical center policies on scheduling, overtime, sick time and time off. 23 Arrives punctually, takes appropriate time for breaks, use sick time appropriately. Qualifications QUALIFICATIONS/JOB REQUIREMENTS: EDUCATION: Graduation from an accredited school of professional nursing. Baccalaureate degree in Nursing preferred. EXPERIENCE: Staff Nurse II – Minimum of six months Registered Nurse experience. Six months current ER & triage experience or completion of Seton ER Nurse Training Program. CERTIFICATIONS/LICENSURE: Active California Registered Nurse licensure Basic Life Support certification (BLS) required ACLS certification required PALS certification required NIHSS certification required Workplace Violence Prevention De-escalation 8-hr (AB508) certification or CPI certification required Preferred: NRP certification. OTHER SKILLS, ABILITIES & KNOWLEDGE: Diffusing Assaultive Behavior Training Knowledgeable of the California Nursing Practice Act. Able to understand and follow all policies, procedures, guidelines, rules and regulations of Nursing Service, Seton Medical Center, Title XXII. Excellent communication and customer service skills. Proficient in Evident computerized documentation system preferred.
