This position represents an amazing opportunity for an endocrinology or diabetes specialized nurse practitioner (APRN) with a big heart and entrepreneurial spirit. Your primary focus will be delivering world class diabetes care to our members. In this role you will be working in a next generation virtual diabetes clinic that dramatically expands access to specialty care for America's most vulnerable seniors. You will also utilize and help improve our home-grown technology and electronic health information platform to carry out these visits. On a day-to-day basis you will work closely with our virtual speciality clinic team members at Devoted Medical including Certified Diabetes Care and Education Specialists (CDCES), physicians and other APRNs, medical assistants, pharmacists and social workers. You will be a key member of our interprofessional team.

The diabetes clinic is one of several of Devoted's virtual specialty care programs that were designed as “micro centers of excellence” that deliver highly tailored, specialized care to patients with a specific chronic condition - e.g. diabetes, congestive heart failure, hypertension, etc.

Responsibilities will include:

  • Conduct focused and thorough assessments of patients with diabetes through virtual consultations including ordering advanced diagnostics, interpreting lab and imaging data, and developing a treatment plan in collaboration with the speciality care clinic team.

  • Formulate accurate diagnoses and develop individualized treatment plans for patients with diabetes, including medication management, lifestyle modifications, and monitoring recommendations.

  • Collaborate closely with other members of the care team including PCPs, endocrinologists, and other Devoted team members including pharmacy, clinical nursing, social work, certified diabetes educators, as well as interfacing with family members and caregivers to coordinate holistic care for the member, ensure continuity of care and deliver a collaborative care plan.

  • Serve as the clinical advisor and provide clinical escalation support for the speciality clinic staff and other teams during business hours.

  • Participate in regular panel review discussions to offer advice and provide guidance around medical management.

  • Perform comprehensive assessment including comprehensive diagnosis/disease review, medication review, and assessment for quality of care (STARS/HEDIS) interventions as well as social and home health/DME needs.

  • Utilize our home grown electronic health information system for visits while also providing feedback on how to improve the interface.

  • Maintain accurate and up-to-date patient records, ensuring compliance with relevant legal and ethical guidelines.

  • Participate in quality improvement initiatives and ongoing professional development to stay current on best practices and advancements in diabetes care.

  • Adhere to all relevant laws, regulations, and industry standards, including patient privacy and telehealth regulations.

Desired skills and experience:

  • APRN with 5 or more years working in outpatient clinical practice ideally in an endocrinology or diabetes focused clinic.

  • Minimum of 2 years of experience concentrated in diabetes management and education required.

  • Proficiency in using telehealth technology and electronic health records (EHR).

  • Virtual care experience is preferred along with a strong desire to continue practicing clinical nursing and performing virtual visits - you believe in the mission of bringing care to where the patient lives.

  • An understanding of managed care is a plus, including how to appropriately assess STARS/HEDIS measures, code clinical comorbidities, and identify clinical care gaps.

  • Proficiency in English and ideally Spanish required for this position.

  • Multi-state licensure is required in addition to a willingness to obtain, and maintain, additional licensure as requested.

Licensure and Certification:

  • Master's or Doctoral degree in Nursing with a specialization in diabetes care.

  • An active and clear RN and APRN license as well as APRN certification is required at time of hire and must be maintained while employed at Devoted Medical.

  • Active BLS is required at time of hire and must be maintained while employed at Devoted Medical.

Salary Range: $120K-155K base range plus performance based bonus paid out quarterly or annually ($10K-$20K) for a total comp range of $130K-170K

Advanced Practice Provider NP/PA - Transitions of Care

Job Description Schedule: This is a full-time, 5-day, 8-hour position (8:30 AM - 5:00 PM; MST/PST preferred ), including one weekend shift per month. Candidates must be willing and able to consistently work within Mountain Standard Time (MST) or Pacific Standard Time (PST). A bit about this role: This position represents an amazing opportunity for an Advanced Practice Provider (NP/PA) to help build Devoted Medical’s Transitions of Care (TOC) Program. The TOC program provides virtual care in the 30 day window after a patient is discharged from the acute setting (hospital or skilled nursing facility). The clinical team is highly interdisciplinary and composed of coordinators, guides, nurse case managers, nurse practitioners, and a medical director. The program’s goal is to prevent readmissions by providing exceptional, comprehensive wrap-around care in the post-discharge period. The TOC APP will provide virtual clinical care for a wide range of diseases and across a wide spectrum of acuity. This position will require the ability to conduct a comprehensive post-discharge medical assessment, develop individualized care plans, and provide disease-specific clinical education regarding management and red flags. The TOC APP must also be comfortable diagnosing, managing, and monitoring decompensating patients with common readmission conditions such as heart failure, COPD, diabetes, hypertension, pneumonia, cellulitis, and urinary tract infections. The role will require significant interdisciplinary collaboration with nurse case managers, clinical guides, and clinical coordinators, all aimed at providing the best post-discharge care for patients possible. The TOC APP will also help enhance the team culture of the program by fostering a safe environment for team communication and patient escalations. Your Responsibilities and Impact will include: Perform initial TOC visits. These visits include comprehensive post-discharge assessment of patients’ medical conditions, medications, functional status, and psychosocial needs. Additional requirements include completing a post-discharge medication reconciliation, placing appropriate orders (prescriptions, labs, imaging, referrals, home health, DME), providing appropriate disease education, communicating contingency plans for worsening symptoms, and comprehensively assessing a patient’s medical conditions. Perform acute symptom TOC / virtual urgent care visits. These are visits focused on addressing a patient’s acute need or new symptom. During these visits, NPs will be required to diagnose and manage the patient, and ensure they receive the appropriate level of care. Participate in interdisciplinary TOC rounds. Transitions of Care Advanced Practice Provider Manager

Job Description A bit about this role: This position is an amazing opportunity for an ambitious nurse practitioner to take on a new leadership role in the Transitions of Care (TOC) Program and to help develop the clinical excellence of the team. The TOC APP Manager will be responsible for the clinical and operational oversight of the APPs in the ever-growing TOC program, while continuing to provide clinical care to patients. The position will join the TOC management team (Medical Director, APP Managers, Operations Leads, Nurse Director & Managers, and Staff Supervisors) and together work to strengthen the program’s collaborative culture, empower the front line staff to achieve clinical excellence, and surface opportunities to improve patient care. The Hypertension clinic is one of several of Devoted’s virtual specialty care programs that are designed as “micro centers of excellence” that deliver highly tailored, specialized care to patients with specific chronic conditions. Responsibilities will include: Conduct focused and thorough assessments of patients with hypertension through virtual consultations including ordering diagnostics as needed, interpreting labs and imaging data, and developing a treatment plan in collaboration with the specialty care clinic team. Formulate accurate diagnoses and develop individualized treatment plans for patients, including medication management, lifestyle modifications, and monitoring recommendations. Mitigate the risk of cardiovascular disease by proactively initiating and managing statin therapy in persons with diabetes and/or cardiovascular disease Collaborate with an interdisciplinary care team—including primary care providers, specialists, and Devoted team members such as pharmacy, social work, and health coaches—as well as family members and caregivers to coordinate holistic, patient-centered care, ensure continuity, and implement a collaborative care plan. Serve as the clinical advisor and provide clinical escalation support for the speciality clinic staff and other teams during business hours. Participate in regular panel review discussions to offer advice and provide guidance around medical management. Utilize our home grown electronic health information system for visits while also providing feedback on how to improve the interface. Maintain accurate and up-to-date patient medical records, ensuring compliance with relevant legal and ethical guidelines. Participate in quality improvement initiatives and ongoing professional development to stay current on best practices and advancements in hypertension and atherosclerotic cardiovascular disease risk reduction. Adhere to all relevant laws, regulations, and industry standards, including patient privacy and telehealth regulations. Job Description A bit more about this role: This position is an amazing opportunity for an ambitious nurse practitioner (NP) to take on an impactful leadership role in the Devoted Medical Specialty Department to help develop the clinical excellence of the team. The Specialty Care NP Manager will be responsible for the clinical and operational oversight of the NPs in the Heart Center (CHF and Virtual Cardiology Clinics) and the CKD Clinic while continuing to provide clinical care to patients in either the Heart Center or CKD Clinic. The position will join the Specialty Care management team (Medical Director, Clinic Physicians, Operations Leads, Clinic Managers) and together work to strengthen the programs’ collaborative culture, empower the front line staff to achieve clinical excellence, and surface opportunities to improve patient care. This manager reports to the Medical Director for Specialty Care. Both the Heart Center and the CKD Clinic are Devoted Medical’s newest specialty care clinics focused on delivering high quality, high value, and accessible virtual first heart and kidney care to its members. Responsibilities will include: 50% of your time will be dedicated to providing patient care either in the heart center or the CKD clinic (18 hours per week): These responsibilities include: Conduct focused and thorough assessments of patients with conditions that impact heart or kidney health through virtual consultations including ordering diagnostics as needed, interpreting labs and imaging data, and developing a treatment plan in collaboration with the clinic team. Work with interdisciplinary teams including lead clinic physicians, social workers, pharmacists, and nurses. 50% of the time will be dedicated to serving as the NP manager of the Devoted Heart Center. Your team will consist of those NPs who work in the Heart Center (CHF and Virtual Cardiology Clinics) as well as the CKD Clinic. These responsibilities include: Provide oversight and accountability for the clinical performance of the NP team, quality chart reviews, coaching and teaching. Lead and maintain a strong team culture, being the standard-bearer for a collaborative, unified, engaged provider group. Serve as the liaison to the Clinic Managers and Medical Director on issues regarding patient care, interdisciplinary coordination, and performance management challenges Lead hiring, interviewing, onboarding and ongoing training of the NP team. Oversee licensure of all direct reports to ensure adequate clinical coverage across all markets in all clinical programs. Participate in all ongoing clinical leadership meetings, including interdisciplinary team meetings, clinical oversight meetings, Specialty Care leadership huddles, etc. Identify areas for improvement and implement strategies to achieve targeted metrics.
