This position is responsible for supporting coordination of care for Early, Periodic, Screening, Diagnostic & Treatment (EPSDT) aged members including prevention interventions, coordination, data collection, measurement of EPSDT services and exceeding EPSDT quality and performance measure goals. This position is responsible for ongoing improvement activities associated with EPSDT eligible members, and reports to the Clinical Quality manager.
If you are located in Arizona, you will have the flexibility to work remotely* as you take on some tough challenges. This is a full time role based in Arizona with some travel required to the Phoenix office based on business needs.

Primary Responsibilities
- Ensure members receive EPSDT services and promote preventive health strategies
- Promote access to oral health care services
- Identify and coordinate assistance for identified member needs
- Serve as a liaison with community partners and participate in cross-functional meetings
- Support health plan coordination actions related to Arizona Early Intervention Program (AzEIP) for children with developmental delays or disabilities
- Coordination of care for EPSDT aged members, keeping track of the members' annual well visits and immunizations and identifying and implementing interventions to address identified opportunities for improvement
- Become a subject matter expert on EPSDT and AzEIP activities for the health plan
- Coordinate care for EPSDT members with elevated blood lead levels and ensure timely communication with state regulatory agencies
- Support and monitor health plan responsibilities associated with the Vaccines for Children (VFC) program
- Ensure timely submission of all state EPSDT reporting requirements
- Assist Clinical Program Manager/QM Associate Director in meeting regulatory quality standards
- Prepare and deliver formal presentations to committees or work groups
- Work independently on assigned projects under the supervision of the QM Manager and/ or QM Associate Director
- Develop provider and member education and training materials related to health promotion
- Assists with the maintenance of Quality policies & procedures; makes recommendations to other departments for the development of policies and procedures to meet the quality goals of the plan
- Solid problem-solving, organizational, and communication skills
Required Qualifications
- Current, unrestricted RN license in the state of Arizona
- 2+ years of healthcare experience
- Proficiency with Word, and Excel
- Reside in Arizona
- Master's degree in health services, public health, healthcare administration, or related field
- Certified Professional in Healthcare Quality (CPHQ) or Certified in Health Care Quality and Management (CHCQM)
- 2+ years of experience with pediatric or family practice nursing
- Experience with Early, Periodic, Screening, Diagnostic & Treatment (EPSDT)
- Experience with health plan coordination actions related to Arizona Early Intervention Program (AzEIP) for children with developmental delays or disabilities
- Experience with quality improvement activities
- Experience with Medicaid and/or Medicare health care regulator or insurance organization
