Corewell Health Community Medicine Clinic is looking for a skilled Physician Assistant or Nurse Practitioner to join their busy practice caring for patients across the generational spectrum located in the heart of Grand Rapids. Experienced Preferred.

Interested in working to improve the overall wellness of Grand Rapids' homeless and low-income residents, regardless of resources or ability to pay? This is perhaps our most important service as the health of any community depends on all its residents' health. 

The Corewell Health Medical Group Community Medicine Clinic offers full-service primary care with after-hours on-call nurse triage for urgent needs. We have an integrated behavioral health model to serve populations seeking to access mental health services in the community. Our goal is to help develop patient-centered care plans. We also provide support and education with a nurse case manager who specializes in the management of patients with chronic diseases. 

We support individuals and family members who are struggling with mental health disorders, including substance use disorder and addiction, and help connect patients with substance use disorder services in the community. We provide LGBT-aware care with on-site transgender healthcare. Additionally, we work closely with homeless populations and newly arrived refugees.

Qualifications: 

Nurse Practitioner: 

  • Required Master's Degree Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a master’s, post-master’s or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission 
  • 3 years of relevant experience current, relevant clinical experience 
  • Previous experience functioning in a collaborative role as a Nurse Practitioner 
  • LIC-Nurse Practitioner (NP) - State of Michigan  
  • CRT-Basic Life Support (BLS)  
  • CRT-Neonatal Resuscitation Program (NRP)
