Schedule:
- Full-Time Day Shift
- 40 Hours Per Week
- 8-4pm or 9-5pm
Key Responsibilities:
- Serve on a team to coordinate care for newly diagnosed thoracic and head & neck cancer patients, guiding them through their cancer journey with compassion and expertise.
- Serve as a liaison for patients who are seeking a second opinion in their cancer journey.
- Timely Access: Ensure patients receive prompt access to vital diagnostic tests and treatments, facilitating a smooth care experience. Serve as a conduit and clinical connection between medical specialists, surgical oncology, medical oncology, radiation oncology and all ancillary care providers (such as Nutrition, Palliative Care, Mental Health Services, etc.).
- Collaborative Coordination: Work closely with our dedicated teams of nurses and physicians to develop and implement personalized treatment plans.
- Innovative Contributions: Engage in tumor board discussions, helping to shape innovative treatment strategies for complex cases.
- Seamless Communication: Foster effective communication between nursing and physician teams to optimize patient care.
- Facilitate patient education and connectivity to academic, tertiary services such as clinical trials and novel therapeutics.
- Be at the forefront of thoracic and head & neck cancer care working with advanced technologies and treatment options.
- Play a crucial role in enhancing patient outcomes and quality of life through effective coordination.

Description:
Knows and uses the nursing process in planning, implementing, and evaluating nursing care to individual patients. Collaborates with other members of the healthcare team. Evaluates response to interventions and patient status toward goal achievement. Accepts delegated nursing measures for which he or she knows he or she is prepared, qualified and licensed to perform.
- Requires graduation from a nationally accredited licensed practical nursing program.
- Requires an active Licensed Practical Nurse (LPN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) LPN license.
- Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
- Successful completion of orientation required.
- 1-2 years of relevant experience preferred.
