  • JOB SUMMARY
    • Promotes and restores patients' health by completing the nursing process; collaborating with physicians and interdisciplinary team members; providing physical and psychological support to patients, friends, and families; delegate and evaluate care assigned to LPNs and CNAs to meet patient care needs.
  • ESSENTIAL FUNCTIONS
    • IDENTIFIES PATIENT/RESIDENT CARE REQUIREMENTS
      • By establishing personal rapport with potential and actual patients/residents and other persons in a position to understand care requirements.
      • Uses the nursing process as a framework to deliver care by establishing or contributing to a plan of care with the patient/resident including a teaching plan, within 24 hours of admission;
        • establishing or contributing to prioritization of goals appropriate to the patient’s/resident's age and ability to learn;
        • estimated length of stay, medical diagnoses and discharge plan;
        • identifying nursing care needs, writing nursing interventions and identifying educational objectives appropriate to care standards;
        • by reassessing and documenting the effectiveness of nursing actions and the patients/residents progress toward goals as the patient’s/resident's condition changes;
        • by communicating the plan and patient/resident progress to the interdisciplinary team.
        • delegating the plan of care to other licensed and unlicensed associates within their scope of practice
    • ESTABLISHES A COMPASSIONATE ENVIRONMENT
      • By providing emotional, psychological, and spiritual support to patients/residents, friends, and families.

      • By establishing patient/resident care goals;
        • educating patient/resident, friends, and family to understand condition, medications, and self-care skills;
        • answering questions by applying rehabilitation principles to patients/residents served;
        • by demonstrating competence in providing age specific care to pediatric patients/residents, adult patients and geriatric patients/residents.
    • ASSURES QUALITY OF CARE
      • By adhering to therapeutic standards;
        • measuring health outcomes against patient/resident care goals and standards;
        • making or recommending necessary adjustments;
        • following hospital and nursing division's philosophies and standards of care set by state board of nursing, State Nurse Practice Act, and other governing agency regulations;
        • by participating in performance improvement activities;
        • by assessing care provided to assigned patients/residents by LPNs, Nursing Assistants, agency personnel, and other associates.
    • RESOLVES PATIENT/RESIDENT PROBLEMS AND NEEDS

  • QUALIFICATIONS

      • Education
        • Associate's Degree in Nursing or Nursing Diploma required
        • Bachelor's Degree in Nursing preferred
      • Work Experience
        • Previous med-surg experience or rehabilitation experience preferred
        • 1-2 years of intensive care or telemetry experience preferred
      • Licenses / Certifications
        • Registered Nurse license required
        • Basic Life Support / CPR, as a healthcare provider, per the American Heart Association required
        • Advanced Cardiac Life Support/ACLS (must be acquired within the first 6 months of employment).
        • PALS certification preferred for all staff (must be acquired within the first 6 months of employment).

Provides evaluation of needs of individual care planning information as necessary and as requested by RN Supervisor, Director of Nursing, Assistant Director of Nursing and other health care disciplines; completes individual education in accordance with established procedures. Performs all position duties according to safety rules and standards of practice required for this position, and as required to ensure the general safety of staff, individuals and visitors of the facility. Reports to the Director of Nursing, Assistant Director of Nursing, RN Supervisor and/or licensed professional any unusual symptoms, changes of condition, daily needs and progress of individuals. Contacts MD/NP/PA for orders or updates according to individual’s response to treatment plan and/or situation and updates family members as needed. Participates in Change-of-Shift Report and is responsible for bringing nursing personnel of next shift up to date regarding the care and needs of individuals. Attends and participates in orientation and in-service training prior to performing tasks that are new or that the RN does not feel competent performing. Attends and participates in continuing education programs designed to keep staff abreast of changes in the profession, changes in company policies/procedures, and completion of all annual mandatory education. Encourages and participates in the ongoing education and professional development of NTs and LPNs. Serves as a role model and represents the organization in a manner that conveys professionalism, appearance, confidentiality, courtesy, conduct, honesty, fairness, personal integrity and a respect for the fundamental rights, dignity, personal comfort and privacy of others. Participates in quality improvement, clinical and safety programs upon request. From Up to Qualifications Educational Requirements and Qualifications for Registered Nurse (RN) (Pediatrics): Credentials as a Registered Nurse with a current state license in good standing for the state in which work is performed Valid, current BLS/CPR and First Aid certification Must be at last 21 years of age Training and work experience in a pediatric setting preferred Meets the qualifications of a Child Care Supervisor as described in 55 Pa.
