Performing ongoing assessments and revision of the plan of care, with interdisciplinary collaboration, in response to the changing needs of the patient and family/caregiver; 5. Performing comprehensive assessment of pain/symptoms and developing individualized symptom management plan; 6. Anticipating, preventing, and treating undesirable symptoms; 7. Providing support, instruction, and education to the patient/family/caregivers; 8. Documenting nursing assessments, identified problems, measurable goals of care, limitations to provision of care, care interventions and response to care; 9. Coordinating all patient and family/ caregiver services with the hospice interdisciplinary team and other health care providers; 10. Consulting and collaborating with the hospice interdisciplinary team and others involved with the patient’s care; 11. Developing the hospice aide plan of care and providing direct and indirect supervision of aide at least every 14 days; 12. Maintaining the dignity of the patient; 13.
