Position Overview:

The CNA is a member of the community nursing team whose responsibility is to assist professional nursing personnel by delivering direct hands-on nursing care to ambulatory and non-ambulatory residents on a daily basis. The CNA is also responsible for assisting the residents to achieve the highest level of functioning while living as normal a lifestyle as possible.

Key Responsibilities:

• To perform or assist the resident with completing Activities of Daily Living (ADL).

• Respond to residents call lights to provide maximum comfort, safety and privacy.

• Transport residents by ambulation or by wheelchair to areas of community (dining room, lobby and activities).

• Report any significant findings/observations to nurse immediately.

• Assist residents on admission, transfer and discharge to make a smooth transition.

• Responsible for the safekeeping of residents’ personal possessions.

• Keep resident rooms neat and safe.

• Follow proper infection control techniques

• Respect the rights of each resident, particularly privacy and confidentiality.

• Re-teach resident to perform ADL skills.

• Provide restorative care to prevent pressure sores, contractures, and complications.

• Participate in Continuous Quality Improvement and ensures follow-up occurs.

• Converse with resident and family; assist them on adjustment to the community.

• Ensure adequate hydration and nutrition of resident; report deviations from norm to the Charge Nurse promptly.

• Perform blood pressure, pulse, respirations, intake/output and weights accurately.

Qualifications:

• Certified and state tested as a Certified Nursing Assistant in the state employed; certificate must be active, valid, and in good standing.

General Description of the Job Class The Nursing Assistant (NA) is a patient care support partner with the Registered Nurse (RN) and/or Licensed Practical Nurse (LPN) and performs work as delegated by the RN or LPN. The NA provides direct care to patients and assists with non-direct patient care unit coordination. The individual in this position is expected to perform basic care and comfort standards as well as have knowledge of the patient population needs in the assigned work area.
