Summary
LiveWell Partners’ OASIS Coordinator is responsible for overseeing OASIS documentation accuracy, consistency, and compliance to support quality outcomes and regulatory readiness. This role also manages relationships with outsourced OASIS review vendors and leads the resolution of clinical documentation exceptions.
Primary Responsibilities:
• Review Start of Care (SOC), Resumption of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments for accuracy, consistency, and alignment with patient condition and plan of care.
• Oversee the performance of outsourced OASIS reviewers, ensuring alignment with agency expectations and timely turnaround.
• Manage the clinical documentation exception process; follow up with clinicians and supervisors to resolve discrepancies or missing data.
• Provide clinical coaching or feedback to field staff and supervisors on documentation gaps or improvement opportunities.
• Monitor OASIS-related metrics, identify adverse trends, and collaborate with managers to implement improvement plans.
• Participate in QAPI, clinical performance improvement, and compliance activities.
• Other duties as assigned.
Qualifications:
• Current registered or licensed clinician with license in good standing.
• 2+ years of clinical home health experience.
• Strong working knowledge of OASIS, documentation standards, and care planning.
• COS-C or HCS-O certification preferred (or willingness to obtain).
• Knowledge of federal and state home health regulatory requirements.
• Exceptional attention to detail and critical thinking skills.
• Effective communicator and collaborator across multidisciplinary teams.
