Key Responsibilities:

Administer medications safely and accurately, adhering to state and company guidelines.
Provide health consultations, effectively communicating any changes in residents’ health to physicians and families.

Maintain meticulous documentation in compliance with departmental standards.

Conduct thorough resident assessments as required, upholding confidentiality standards.

Qualifications:

Associate Degree in Nursing from an accredited program is preferred.
Current, unencumbered LPN license in the state of Virginia.
BLS and CPR certification required.
Previous experience in a clinical setting is preferred.
Assisted living experience is preferred.

Supervise, train, and support care team members, promoting teamwork and adherence to care standards. Assist in the transcription of physicians' orders and the review of new physicians' orders. Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation. Coordinate care planning with home health agencies on-site, working within community and company policies. Address residents’ special needs, requests, and complaints. Coordinate admissions with the sales team, including supervising move-ins. Understand the community's care regulations along with state-specific regulations. Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery. Requirements CANDIDATE QUALIFICATIONS Education and Certifications: A valid, incumbent nursing license (LPN/LVN) is required. High School Diploma or equivalent. An associate degree or higher in nursing is preferred, or a combination of education and experience. Additional coursework in management is an asset. Current First Aid and CPR license or ability to obtain. Experience, Competencies, and Skills: At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies. At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred. Experience with memory care is a plus.
