Qualifications:

- Graduation from an accredited nursing school and valid nursing license in the state(s) of practice in Montana.

- Preferred: Two (2) years of nursing experience, with at least one (1) year in home health.

- Commitment to the agency’s philosophy and goals.

- Ability to exercise independent judgment.

Principal Responsibilities:

Reporting to the Home Health Director and Clinical Manager, you will:

- Conduct initial evaluations and regularly reassess patients’ nursing needs, initiating and updating personalized care plans.

- Provide specialized nursing services, including medication administration and wound care.

- Implement preventive and rehabilitative procedures to promote patient wellness.

- Prepare accurate clinical documentation and progress reports per agency policy.

- Coordinate patient services and maintain schedules.

- Communicate any changes in patient conditions with the healthcare team.

- Educate patients and their families about care plans and health management.

- Participate in in-service training and foster team development. 

-Participate in Interdisciplinary home health care meetings weekly

- Adhere to agency policies and efficiently process medical orders.

- Assess nursing needs, refer patients to specialists as necessary, and supervise other nursing staff and visits.

- Lead patient care conferences and participate in Quality Assurance and Performance Improvement initiatives.

- Provide comprehensive patient care at home and be available for on-call house calls- as assigned.

- Complete and submit OASIS assessments and documentation within the required timeframes.

Preferred Qualifications

  • Home care and home health background.

  • Experience working in a home healthcare environment.

  • Familiarity with healthcare regulations, HIPAA compliance, and patient privacy requirements.

  • Knowledge of the local healthcare market and community outreach strategies.

  • Knowledge of appropriate software including Microsoft Word, Excel, Outlook, Wellsky (EHR).

  • Knowledge of office management systems and procedures.

  • Clean driver’s license with a clean driving record (Preferred).
