Position Summary

The RN/LPN participates in the delivery of patient care in accordance with facility clinical systems, regulatory requirements, and physician orders. This role includes overseeing CNAs to ensure the highest quality of care is provided in a supportive, resident-focused setting.

Qualifications

Current and unrestricted Illinois RN or LPN license
CPR certification preferred (not required)
Strong verbal and non-verbal communication skills
Skilled nursing facility experience preferred

Key Responsibilities

Provide and oversee care to ensure resident health, safety, and well-being
Direct, support, and mentor CNAs on your team
Assess resident needs and develop individualized care plans
Identify and communicate changes in resident condition to physicians, NPs, staff, and families
Administer medications, treatments, and direct care as ordered
Coordinate resident admissions, transfers, and discharges
Ensure smooth shift transitions through effective communication
Document care accurately in PointClickCare (EHR)
