Position Summary:
The Licensed Practical Nurse (LPN) will be responsible for providing direct patient care under the supervision of a Registered Nurse (RN) and in accordance with the policies and procedures of Ditmas Park Dialysis Center. The LPN will play a critical role in the daily operations of the dialysis unit, ensuring the safety and well-being of our patients while maintaining a high standard of care.
Key Responsibilities:
- Patient Care:
- Monitor vital signs.
- Prepare patients for dialysis treatment, including cannulation of access sites.
- Administer medications and IV fluids as prescribed.
- Monitor patients during dialysis treatment, addressing any complications or concerns.
- Provide patient education on dialysis care, diet, and medications.
- Assist in the development and implementation of individualized patient care plans.
- Equipment and Safety:
- Operate and maintain dialysis machines and equipment.
- Ensure the proper setup and breakdown of dialysis machines.
- Follow infection control and safety protocols to prevent cross-contamination.
- Conduct routine checks on equipment to ensure functionality and safety.
- Documentation:
- Maintain accurate and detailed patient records.
- Document patient progress, treatment plans, and any changes in condition.
- Ensure all documentation complies with regulatory and organizational standards.
- Team Collaboration:
- Work closely with RNs, nephrologists, dietitians, and other healthcare professionals to provide comprehensive patient care.
- Participate in team meetings and contribute to continuous improvement efforts.
- Assist in training and mentoring new staff members.
- Quality Improvement:
- Participate in quality improvement initiatives to enhance patient care and operational efficiency.
- Report any incidents, errors, or safety concerns promptly to the Nurse Manager.
Qualifications:
- Education:
- Graduate of an accredited LPN program.
- Licensure:
- Current and valid LPN license in the state of New York.
- Experience:
- Previous experience in a dialysis or acute care setting preferred.
- Skills:
- Strong clinical and assessment skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
- Proficiency in electronic health records (EHR) systems.
- Certifications:
- Basic Life Support (BLS) certification required.
- IV Certification preferred.
Physical Demands and Work Environment:
- Frequent standing, walking, bending, and reaching.
- Exposure to bloodborne pathogens and infectious diseases.
- Use of personal protective equipment (PPE) as required.
