Experienced Dialysis LPN

Position Summary:

The Licensed Practical Nurse (LPN) will be responsible for providing direct patient care under the supervision of a Registered Nurse (RN) and in accordance with the policies and procedures of Ditmas Park Dialysis Center. The LPN will play a critical role in the daily operations of the dialysis unit, ensuring the safety and well-being of our patients while maintaining a high standard of care.

Key Responsibilities:

• Patient Care:
  • Monitor vital signs.
  • Prepare patients for dialysis treatment, including cannulation of access sites.
  • Administer medications and IV fluids as prescribed.
  • Monitor patients during dialysis treatment, addressing any complications or concerns.
  • Provide patient education on dialysis care, diet, and medications.
  • Assist in the development and implementation of individualized patient care plans.
• Equipment and Safety:
  • Operate and maintain dialysis machines and equipment.
  • Ensure the proper setup and breakdown of dialysis machines.
  • Follow infection control and safety protocols to prevent cross-contamination.
  • Conduct routine checks on equipment to ensure functionality and safety.
• Documentation:
  • Maintain accurate and detailed patient records.
  • Document patient progress, treatment plans, and any changes in condition.
  • Ensure all documentation complies with regulatory and organizational standards.
• Team Collaboration:
  • Work closely with RNs, nephrologists, dietitians, and other healthcare professionals to provide comprehensive patient care.
  • Participate in team meetings and contribute to continuous improvement efforts.
  • Assist in training and mentoring new staff members.
• Quality Improvement:
  • Participate in quality improvement initiatives to enhance patient care and operational efficiency.
  • Report any incidents, errors, or safety concerns promptly to the Nurse Manager.

Qualifications:

• Education:
  • Graduate of an accredited LPN program.
• Licensure:
  • Current and valid LPN license in the state of New York.
• Experience:
  • Previous experience in a dialysis or acute care setting preferred.
• Skills:
  • Strong clinical and assessment skills.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team-oriented environment.
  • Proficiency in electronic health records (EHR) systems.
• Certifications:
  • Basic Life Support (BLS) certification required.
  • IV Certification preferred.

Physical Demands and Work Environment:

• Frequent standing, walking, bending, and reaching.
• Exposure to bloodborne pathogens and infectious diseases.
• Use of personal protective equipment (PPE) as required.
