SYNERGY HomeCare is looking for compassionate and dedicated Registered Nurses (RN) to join our team.

Registered Nurse (RN) Responsibilities:

-Follow a designated care plan in accordance with patient's needs

-Make home visits to clients in designated geographic territories

-Perform assigned duties, including administration of medication, wound care, treatments, and procedures

-Monitor clients' conditions; reporting changes to Clinical or Client Services Manager

-Follow up with, execute, and properly document doctors' orders

-Perform client assessments as necessary

-Case management and coordination

-Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.

Qualifications for a Registered Nurse (RN):

-Graduation from an accredited and approved nursing program
-A current license as a Registered Nurse in New Jersey (NJ)

Overnight per diem role (7:00PM to 7:30AM) Per diems must work at least 6 shifts per month Job Details Our Hospice RN’s: The Inpatient Hospice RN is responsible for providing nursing care to hospice patients, residents and/or family in our 12 bed unit located in Rockville Center, inside Mercy Hospital. Patient care focuses on on symptom management and pain control. Our ideal candidate will have knowledge of hospice concepts, philosophy, policies and procedures. Prior experience in home care, hospice and/or oncology preferred. Qualifications: Current and valid NY State license to practice as a Registered Nurse. One to two years recent general bedside nursing experience with good clinical skills.
