Addiction Treatment Center - Claremont, NH

40 hours per week. Monday-Friday, 8am-5pm. 

 

The Clinical Nurse is an engaged and credentialed member of the Professional Nursing Organization and is responsible for autonomous practice directed by the professional tenets of practitioner, leader/decision maker, scientist and transferor. The Clinical Nurse is responsible for utilizing the nursing process to provide evidence-based care and to continuously monitor and evaluate practice to ensure safe passage of patients that is in the best interest of populations served.

Qualifications

Description Job Summary: Primary responsibility is to provide strong, prioritized patient care in a variety of urgent or emergent situations while maintaining a global perspective of the department. Qualifications: Current and maintained Paramedic Certification or Critical Care Paramedic level or graduate of an accredited school of nursing, BSN preferred. Current Vermont RN license in good standing. Two-three years’ experience in Emergency Nursing. Current BLS, ACLS, PALS, required. TNCC, ENPC, and CEN preferred. RN Case Manager Hospice - Visiting Nurse and Hospice

Overview At the direction of the Clinical Manager, coordinates/manages a case load comprised of Hospice patients. Assesses, plans, implements, coordinates, and evaluates care for each patient/family in conjunction with the hospice interdisciplinary team. Case management includes but is not limited to; comprehensive needs assessments, development of appropriate plan of care, referral to internal and external resources, evaluation of outcomes, and provision of nursing educational, clinical and related services. Supervises the Licensed Nursing Assistants/ Home Health Aides. Responsibilities Provides a full range of professional nursing care to patients in their homes under the medical direction of a licensed physician. Regularly reevaluates patient's nursing needs and informs physician and other personnel of any changes in patient's condition or needs. Initiates the plan of treatment and any necessary revisions. Supervises the Home Health Aides. Teaches nursing care to be given in the home and reviews and explains the patient care plans. Performs initial and subsequent assessments of patient/family physical, emotional, psychosocial, spiritual and cultural needs to ensure and coordinate the delivery of comprehensive, comfort-focused services through the development of the Hospice patient plan of care. Represents VNH in a positive, proactive manner while participating in community education/support/ PR events.
