Summary:
The LPN Coordinator supports the delivery of high-quality, patient-centered care by combining clinical expertise with operational coordination. This role assists providers in performing healthcare services for patients, oversees daily workflows, and serves as a resource for medical assistants and other LPNs. Responsibilities include direct patient care, medication administration, and coordination of clinical operations to ensure compliance, efficiency, and patient satisfaction.
Accountabilities:
• Clinical Responsibilities:
• Administers prescribed medications, injections, and vaccines; reads PPD results under provider direction.
• Obtains and documents vital signs; reports abnormal findings promptly.
• Performs EKGs and other screening tests; assists with minor surgical procedures while maintaining sterile technique.
• Collects, tests, and prepares specimens for transport; documents results and takes appropriate action for abnormal findings.
• Triages patient calls and escalates to RN or provider as needed.
• Cleans, disinfects, and sterilizes instruments; ensures equipment is in proper working order.
• Maintains medication storage areas and monitors expiration dates.
• Operational Responsibilities:
• Oversees daily schedule, staffing, and workflow for assigned area; communicates changes effectively.
• Acts as liaison between staff and management; disseminates information and escalates issues as needed.
• Monitors real-time performance metrics and provides coaching; may gather input for performance evaluations.
• Participates in interviewing and onboarding new staff; provides orientation and training support.
• Ensures compliance with regulatory standards (OSHA, HIPAA, TJC, etc.) and practice policies.
• Maintains inventory of clinical supplies and coordinates ordering.
• Serves as EMR resource for staff and supports accurate documentation.
• Patient Care Coordination:
• Facilitates communication with specialists, ancillary services, and other health providers.
• Prepares paperwork and documentation for patient visits; manages test results and referrals.
• Assists in resolving patient concerns or complaints in collaboration with management.
• Performs duties in accordance with Penn Medicine and entity values, policies, and procedures

Credentials:
- Licensed Practical Nurse (Required)
- Basic Cardiac Life Support (Required)
- CPR/BLS certification (within 30 days of hire).
Education or Equivalent Experience:
- H.S. Diploma/GED (Required)
- Completion of accredited Practical Nursing Program. 2+ years Outpatient clinical experience preferred
