Responsibilities of an LPN:

Credentials: BLS/CPR, as a healthcare provider as per the American Heart Association required. Summary: The Clinical and Scheduling Operations Assistant provides administrative and clinical support to teams within home care. The Clinical and Scheduling Operations Assistant is responsible and accountable for accessing appropriate information systems and referral sources to obtain additional information, contacting physicians to obtain/confirm orders, contacting patients/families to provided telephonic initial contact, supports the clinical team in the scheduling of home care visits, and assists team managers in coordination of care. Answers phones in a timely manner. Manages/handles patient requests and routes appropriately, enlisting management assistance when appropriate. Gathers, reviews and ensures receipt of essential components of the patient’s chart including consents, NOMNCs, and other regulatory documents. Maintains complete and accurate work queues and responds to work queue issues timely. Maintains clinical competency and able to perform home visits as necessary.
