The Patient Navigator's main function is guiding the cancer patient through the healthcare system serving as an advocate and consistent contact person for the patient and family. The ideal candidate will not only be compassionate but also demonstrate the ability to communicate and work collaboratively with other health care providers. The Patient Navigator will provide support and education as well as timely coordination of care throughout the entire experience. The Navigator must be professional, attentive to detail, and build relationships with the primary care providers as well as the departments of Surgery, Oncology and Radiation Therapy. The Navigator will identify barriers and be the liaison to connect the patient with needed support services. The expectation is that this position will be an active participant with the multi-team committee, monitor quality outcomes, produce audit and tracking information and develop the necessary tools and resources to meet the needs of the cancer patient.

Thoracic Navigator
Duties include
- Help navigate patients who are diagnosed with lung cancer or patients who are suspicious for cancer in the thoracic area.
- Getting the case summaries ready for thoracic tumor board and taking recommendations
- Overview of the lung cancer screening program
Duties include
- Help navigate patients diagnosed with all types of cancers as needed
- Getting the case summaries ready for general tumor board and taking those recommendations
Duties include
- Educating patients on oral chemotherapy/anti-cancer medications and the process for obtaining these
- Works with physicians to order chemotherapy/anti-cancer medications, and lab work and testing as needed
- Help with finding financial assistance with these oral medications if needed
- Clinical monitoring of patients prior to each cycle and other appropriate intervals who are taking these oral medications
Education/Training/Experience
JOB QUALIFICATIONS
Required
- Graduate of an RN School of Nursing
- Excellent written and verbal communication skills
- RN with 3-5 years experience in cancer related field.
- BSN, with 3-5 years experience in nursing with oncology and chemotherapy experience.
- Experience in developing, implementing and evaluating clinical outcomes.
- Oncology background as well as chemotherapy or radiation therapy.
- Possesses current State RN license.
- Chemotherapy certification and Oncology Nursing Certification required within 2 years.
