UNCHMG is looking for a Per Diem APP to join the growing UNC Lenoir’s Transitions of Care Clinic as a virtual APP.

Responsibilities: 

• Provide remote patient management and care, collaborating with healthcare providers. 

• Utilize electronic medical records (EMR) systems to facilitate communication and care delivery. 

• Manage patient communication, including scheduling appointments and sending reminders. 

• Facilitate video consultations and ensure timely responses to patient inquiries. 

Schedule: 1 or 2 days a week Hours: 8am-4pm  (Per Diem)

EMR: EPIC 

Qualifications: 

• Graduation from an accredited NP or PA school 

• One or more years of full panel primary care experienced required 

• North Carolina licensed 

• BLS & ACLS certified

Summary: An Advanced Practice Provider functions as part of the interdisciplinary healthcare team in accordance with privileges approved by the credentialing committee to provide high quality, cost effective care to patients within the APPs scope of practice in collaboration with a supervising physician.
