Responsibilities

The Registered Nurse is responsible for the Shift coordination and oversight of the nursing process for residents assigned to his or her care. Responsibilities include both the direct and indirect provision of care according to each resident's nursing and medical plan of care and in accordance with state and federal regulations and accepted nursing standards of practice. Job duties include, but are not limited to:

  • Provide direct nursing care to assigned residents in accordance with the nursing process.
  • May be assigned to function as Medication/Treatment Nurse, unit Charge Nurse, or Supervisor.
  • Direct day to day functions of Licensed Practical Nurses and Certified Nursing Assistants, assuring compliance with state and federal regulations and facility policies and processes.
  • Provide assessment of and responds to resident conditions utilizing the nursing process.
  • Work cooperatively with other members of the facility interdisciplinary team.
  • Maintain effective communication with other members of the interdisciplinary team utilizing facility processes in order to assure the highest standard of care is maintained for each resident assigned to his or her care.
  • Complete and ensure LPN and C.N.A. documentation of care and services rendered according to facility policies and procedures.
  • Complete reports such as the 24-hour change in condition report, Incident and Accident reports, etc. according to the facility policy and procedure and as assigned by the Director of Nursing Services.
  • Provide immediate response to situations of resident risk such as allegations of abuse, neglect or misappropriation of property.
  • Report incidents timely and to appropriate persons per facility policy.
  • Ensure timely and accurate medication and treatment administration.
  • Communicate to physician and documents changes in resident condition utilizing both objective and subjective nursing assessment.
  • Document and implement changes in nursing and medical plans of care.
  • According to facility policy and procedure, assure that all members of the interdisciplinary team are aware of and understand changes in resident nursing and/or medical plan of care.

Position Requirements

  • Associate's Degree from credentialed school of nursing.
  • Currently licensed as a Registered Nurse in the state of practice.
  • 1 year of related nursing experience

This Command Center Staff Registered Nurse (RN) role is for an RN who wants to be part of an innovative team working as a virtual RN in a command center. The Command Center Registered Nurse will be the nucleus of the care provided to inpatients who are cared for in their home by medical personnel. This RN will have virtual visits with the patient, document in the electronic medical record, coordinate the plan of care, and participate in multidisciplinary rounds. This will all be done from a central command center where you will be working with other RNs and Medical doctors to care for patients who require inpatient care, but can receive the care in their home. Job Description: Reporting Directly to the Virtual Hospital Director of Nursing, The Control Center Staff RN is responsible for providing high-quality patient care to various patient populations within the framework of the Medically Home model. Compensation Pay Range: $70,345.60 - $136,136.00 ESSENTIAL FUNCTIONS Under the direction of the Clinical Manager, evaluates, plans and administers medically prescribed nursing treatment to patients in their home. 1. INITIAL PATIENT ASSESSMENT: Performs a comprehensive physical, psycho-social, and safety assessment of assigned patients as evidenced by clinical documentation. Identifies medical concerns and establishes interventions and goals to achieve measurable outcomes. 2. INITIAL ASSESSMENT DOCUMENTATION: Completes thorough documentation of Admissions, Resumptions of Care, and Second Service Evaluations. A Plan of Care is established identifying realistic and measurable goals. Consistently completes all paperwork required to provide service to patient. Med reconciliation is completed for Agency Admissions, Resumptions of Care, Second Service Evaluations, and with every new medication entered into medical record. Synchronization of all documentation is timely and according to Agency guidelines. 3. REVISITS: TREATMENTS AND DOCUMENTATION: Plans for and provides skilled nursing treatment in the home reassessing patient status on each visit and ensuring quality of care as documented in the clinical record. Treatment plans are modified appropriately to reflect change in patient status and progression towards goals. Pain and vital signs are assessed and documented on every visit. 4. COORDINATION OF PATIENT CARE: Coordinates patient care with other clinical team members ensuring optimal patient care. Communications are documented in the clinical record. MD is notified of any changes in patient status and also when visit frequency is not met per MD order. 5. RECERTIFICATIONS: Agency Recertifications are completed within the 5 days prior to the end of Certification period with thorough completion of OASIS. Recertifications are completed with accurate updated orders documented in the chart. Interdisciplinary communication regarding recertification occurs prior to recertifying patient. 6. DISCHARGES: Discharge planning occurs throughout the episode of care with documentation reflecting that the pt has been prepared for discharge. Discharge is communicated to all disciplines involved and MD is notified of discharge as reflected in the medical record. Discharge summaries are completed and goals and interventions are ended. All required paperwork is appropriately signed by patient/Health Care Proxy indicating notification of discharge. 7. TECHNOLOGY: Demonstrates proficiency and competency in the agency/program automated clinical record as evidenced by successful completion of education program and appropriate daily utilization of software. 8. SAFETY: Fosters a “Culture of Safety” through personal ownership and commitment to a safe environment. Demonstrates proper Infection Control during joint visits. 9.
