Overview

Make a difference for Vermonters with complex health and social needs, working with community based teams and systems, as a RN Nurse for the Vermont Chronic Care Initiative (VCCI) with the Agency of Human Services (AHS). RN Nurse Case Manager Responsibilities:
· Areas include: Patient centered clinical case management, to support health and health related outcomes for individuals with complex health & social needs.
· Community based case management and field based visits with members occur in a variety of settings and may include private apartments, motels, correctional facilities, encampments, and shelters. Individual reports to AHS Field Director and the Central Office Administration and works closely with an array of partners and providers in the health and social services arena, including state partners.
· Service delivery using a team based care approach, patient engagement tools, care teams, shared care plans, a strength based approach and motivational interviewing skills.
· Duties involve need for effective communication and working relationships with a diverse group of stakeholders, including the individual, family members and a number of health care disciplines and service providers; and operational integration with various other health care reform initiatives.
· Clinical experience and knowledge, knowledge of community, and community culture are necessary.

 

Location:
This position primarily covers Windsor County, but flexibility to cover other counties may be requested.

Environmental Factors

Duties are performed in a facility, standard office setting and/or in the community. Extensive travel may be required, for which private means of transport must be available. Certain functions may require duty outside of normal work hours. Some resistance or objection to standards may be anticipated, requiring tact and diplomacy to achieve proper action. Duties involve need for effective communication and working relationships with a diverse group of stakeholders, including the individual, family members and a number of health care disciplines and service providers; and operational integration with various other health care reform initiatives. Critical assessment skills are necessary to promote personal and client safety.

Minimum Qualifications

Possession of (or eligible for) licensure as a Registered Nurse (RN) in Vermont OR eligible to practice in the state of Vermont via a multi-state license AND three (3) years or more of professional nursing experience in an acute hospital setting, long term care, health insurance carrier, or within a community health/public health setting.

 

NOTE: Must maintain Vermont or multi-state licensure as a Registered Nurse as a condition of employment.

Preferred Qualifications

Bachelor of Science in Nursing (BSN).

Special Requirements

Reliable means of personal transportation is required for work out of the office or in the community and field based settings.
