Diabetes Educator Registered Nurse ( RN) - Family Medicine

Overview

Provide patient assessment, teaching and follow up instruction for adult patients with diabetes in the outpatient clinic setting (New London Hospital and Newport Health Center).

Responsibilities

  • Collaborates with primary care providers and teams to support patients with diabetes and their families/caregivers.
  • Serve as coordinator of the American Diabetes Association Diabetes Self-Management Education and Support (DAMES) Program. Collect required data and report as required. 
  • Serve as a coordinator of the CDC - National Diabetes Prevention Program (National DPP). Facilitate, collect required data and report as required. 
  • Coordinate and oversee the planning, implementation, evaluation and effectiveness of the diabetes education program in collaboration with the primary care teams. 
  • Actively participate in quality initiatives related to care of diabetes patients. 
  • Act as a liaison to D-HH for purpose of the ADA-DSMES program.
  • Other duties as assigned. 

Qualifications

  • Bachelor's Degree in Nursing preferred
  • 3-5 years' experience as a Certified Diabetes Educator in the ambulatory setting 

Required Licensure/Certifications

REQUIRED LICENSE(S): • Licensed RN with NH eligibility REQUIRED CERTIFICATIONS: • Certified Diabetes Educator (CDE)
