DEPARTMENT SUMMARY: St. Charles Health System encompasses practices in three Central Oregon counties and numerous specialties including family care, obstetrics, gynecology, pediatrics, immediate/urgent care, cardiology, pulmonology, rheumatology, general surgery, cancer care, and sleep medicine. POSITION OVERVIEW: The Registered Nurse - Primary Care provides quality patient and family centered care as a member of the health care team in accordance with St. Charles Health System’s mission, philosophy, policies, and procedures. The Registered Nurse applies standards for professional nursing practice and focuses clinical care to enhance the experience for patients, families, and others. This position assesses, plans, implements, evaluates, and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. The Registered Nurse is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of the patient. This position may provide direct nursing care to primary care patients, as a member of a primary care team, at one or more SCHS Primary Care clinics.

This position does not directly manage any other caregivers.

ESSENTIAL FUNCTIONS AND DUTIES:

Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural, and disposition planning needs.

Reviews patient history, information and physical with patient/family and assures completion within appropriate timeframe.

Reviews diagnostics and laboratory data and reports abnormal results to the physician and other appropriate caregivers.

Completes assessment and reassessments according to patient need and care team plan of care.

With care team, formulates and implements a plan of care, including follow-up planning, utilizing assessment data and patient, family, and health team input. Ensures goals for patient are realistic, measurable, and developed in conjunction with the patient and family.

Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements.

Delegates appropriately and provides nursing supervision in the provision of care to patients by other members of the care team as appropriate. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patient.

Evaluates the patient's and family's response to care and teaching and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with the care team members.

Documents assessment, planning, implementation, and evaluation in the patient record. Documentation is timely and reflects objective and subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.

Provides care based on the best evidence available.

May participate in the education, role development, and orientation of other personnel as requested. May participate in precepting and mentoring of new staff.

This position may supervise staff and workflow as requested.

Participates in creating a healing environment that supports all aspects of the care environment and the wholeness of each individual, patient and caregiver.

Participates in creating intentional relationships and demonstrates focus attitudes and behaviors that enhance the care experience.

Provides a therapeutic presence in service to others by purposefully responding to the needs of patients in a caring way, including introducing oneself and explaining role in patient’s care, asking the patient his or her preferred name, sitting with the patient to determine his or her care goals, active listening, and communicating effectively and appropriately through touch, eye contact, etc.

Provides and maintains a safe environment for caregivers, patients, and guests

Documents all patient care with proficiency in compliance with hospital policies, procedures, and regulatory agencies. 

Calculates dosages and administers medications accurately in accordance with St. Charles Health System policies and procedures for medication administration.

Develops cultural competence and provides appropriate care to patients and family members who belong to diverse cultural backgrounds.

Supports the vision, mission, and values of the organization in all respects.

Registered Nurse, Transport Team (Everett)

Registered Nurse - Critical Care Transport (0.9FTE Night), Everett. Shifts start at 5pm or 7pm (12 hours). This position is based out of Everett. As an integral member of the Transport Team, the Transport RN functions within the Scope of Practice as defined by Washington State, the ANA Scope and Standards for Pediatric Nursing Practice, and adheres to Seattle Children's Hospital policies/procedures and guidelines of care. Responsible for the safety of patients prior to and during transport to medical care facilities. This includes the provision of basic patient care as well as advanced life support measures required to stabilize the critically ill or injured patient, demonstration of proficiency in neonatal and pediatric critical care nursing and knowledge of the adaptations necessary for patient management in the ambulance or aircraft and during transit. The Transport Nurse must also be familiar with ambulance and aircraft operating procedures and safety practices. Assists in meeting Seattle Children's strategic goals by incorporating the initiatives to support the strategic plans into the daily work, i.e. Continuous Improvement and Innovation (CII), ART, etc. The Transport RN serves as a team lead within the mobile transport team and may be required to provide supervision and delegation to other Registered Nurses, Respiratory Therapists, or Emergency Medical Technicians who are members of the mobile transport team. Must be available for call. This position may be based at Main Campus or at one of the off campus bases. Off campus bases may be at Seattle Children's locations, or at another site that is decided based on volume and efficiency. Required Education and Experience Graduate of an accredited school of nursing; BSN preferred. Minimum of three (3) years recent Critical Care Transport, pediatric critical care (PICU, CICU or other relevant unit), Emergency Department (ED) or neonatal intensive care unit (NICU) experience, at a 6.FTE or greater.
