Per Diem Hospice Registered Nurse (RN) Liaison - The Community Hospice Albany
The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as Liaisons in local hospitals (Capital Region).
Needs are on primarily weekdays - 8am to 4:30pm as needed
Position Highlights:
National pre-taxed mileage reimbursement
Meetings and charting conducted via supplied lap top
Comprehensive orientation provided
What you will do:
- Evaluates patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care.
- Coordinates with the patient’s community physician and our hospice medical directors to confirm eligibility for hospice services.
- Gathers information and ensures a smooth handoff to the hospice RN admitting the patient/family to services.
- Admits to hospice inpatient services upon eligibility, develops a hospice plan of care and collaborates with the hospice interdisciplinary team and the acute care team.
- Works closely with the hospital case management department.
What you will need:
- A current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing, BSN preferred
- 1 year experience as an RN required, preferably in Hospice/Home Care or acute care.
- Hospice or RN Liaison experience a plus but not required.
- The RN must be able to communicate effectively, verbally and written
- Valid Driver's License and Reliable vehicle
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
- Understanding of Hospice principles preferred
Training provided in end of life care.
