The APP will provide high-quality, patient-centered care, focusing on wound management, wound debridement, and support of general surgery workflows. Responsibilities • Perform wound assessments, ongoing wound care, and wound debridement (sharp debridement required).• Manage acute and chronic wounds in collaboration with the General Surgery team.• Conduct patient evaluations, develop treatment plans, and participate in interdisciplinary care planning.• Provide pre- and post-operative surgical care, including follow-up visits and patient education.• Document all care in the EMR in a timely, accurate, and compliant manner.• Coordinate care across both clinical sites, ensuring consistent communication with the surgical team.• Collaborate with physicians, nursing staff, case management, and ancillary services to optimize patient outcomes.
