Position Summary

The Registered Nurse (RN) is responsible for providing direct nursing care to residents and overseeing the delivery of clinical services within the facility. The RN supervises nursing staff, ensures compliance with federal, state, and local regulations, and collaborates with the interdisciplinary team to promote the highest practicable level of resident care, safety, and quality of life. This position functions under the direction of the Director of Nursing.


Essential Duties & Responsibilities

  • Provide direct nursing care in accordance with resident care plans and physician orders

  • Supervise and provide clinical oversight to licensed nurses and nursing assistants

  • Coordinate and manage resident care activities on assigned unit and shift

  • Conduct comprehensive assessments and ongoing evaluations of resident condition

  • Admit, transfer, and discharge residents per facility policies and regulatory requirements

  • Accurately complete and maintain clinical documentation, including assessments, care plans, and nursing notes

  • Receive, transcribe, and implement physician orders in accordance with facility policy

  • Administer medications and treatments safely and effectively

  • Complete incident and accident reports and initiate appropriate follow-up actions

  • Monitor residents for changes in condition and promptly notify physicians and interdisciplinary team members

  • Perform skilled nursing services including wound care, IV therapy (as applicable), catheter care, tube feedings, suctioning, and colostomy care

  • Participate in physician rounds and interdisciplinary care conferences

  • Ensure compliance with infection control, safety standards, and proper use of equipment

  • Maintain resident confidentiality and privacy in compliance with HIPAA and facility policies

  • Promote a calm, respectful, and supportive environment for residents, families, and staff

  • Perform other duties as assigned


Qualifications

  • Current, unencumbered Registered Nurse (RN) license in the state of Illinois

  • Current CPR Certification required

  • Ability to read, write, speak, and understand English

  • Strong clinical judgment and decision-making skills

  • Knowledge of nursing practices, long-term care regulations, and standards of care

  • Prior long-term care experience preferred


Physical Demands

  • Ability to move intermittently throughout the workday

  • Ability to manage the mental and emotional demands of the position

  • Ability to work independently and collaboratively with residents, staff, and external agencies

  • Ability to push, pull, lift, and/or carry a minimum of 25 pounds, up to 5 feet, and for a distance of 50 feet
