Position Summary
The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises the day-to-day nursing activities of nursing assistants. Care is delivered in accordance with current federal, state, and local regulations, facility policies and procedures, and under the direction of the Director of Nursing to ensure the highest practicable level of resident care and quality of life.
Essential Duties & Responsibilities

Direct and supervise daily activities of nursing assistants and assigned staff

Participate in shift planning, including resident care services and activities

Provide leadership and clinical guidance to nursing personnel on assigned unit and shift
-
Admit, transfer, and discharge residents per facility protocol
-
Accurately complete required documentation, including nursing notes and clinical records
-
Receive, transcribe, and implement physician orders in accordance with policy
-
Administer medications and treatments as prescribed
-
Complete incident and accident reports and submit per facility guidelines
-
Monitor resident conditions and report changes promptly to appropriate medical personnel
-
Perform skilled nursing services including catheter care, tube feedings, suctioning, wound care, colostomy care, and range-of-motion exercises

Participate in physician rounds and provide thorough shift-to-shift nursing reports

Promote a calm, respectful, and supportive environment for residents, families, and staff

Ensure compliance with safety, infection control, and equipment-use policies
-
Maintain confidentiality and resident privacy in compliance with HIPAA and facility standards

Perform other duties as assigned
Qualifications
-
Current, unencumbered Illinois Licensed Practical Nurse (LPN) license
-
Current CPR Certification required
-
Ability to read, write, speak, and understand English

Ability to make sound clinical decisions within professional scope of practice
-
Knowledge of nursing practices, long-term care regulations, and clinical procedures
-
Prior long-term care experience preferred

Ability to move intermittently throughout the workday

Ability to manage the mental and emotional demands of the position

Ability to work independently and collaboratively with residents, staff, and external agencies

Ability to push, pull, lift, and/or carry a minimum of 25 pounds, up to 5 feet, and for a distance of 50 feet
