ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Hybrid role supporting PCPs for patient care and managing day to day center operations ensuring that today’s work is done today with special attention to CMAP and HEDIS measures. Supports smooth operations with special attention to scheduling, patient wait times, and other areas of concern to increase efficient and quality services. Assists the Center Clinical Manager in the supervision of Care Promoters and other support staff, including input for performance reviews and corrective action plans as needed. Under the direction of the Center Clinical Manager oversee and train new and existing staff Care Promoters on health education, laboratory, and Medication Room services. Prepares patients for examinations and performs routine screening tests; assists physician with exams; explains treatment procedures and physicians' instructions to patient. Screens walk in patients, fills pill boxes, and provide patient education as requested by PCP. Processes patient phone messages, returning calls and routing them to other team members as appropriate; calls patients to obtain and relay pertinent information for the physician. Upon physician approval and authorization, calls in prescriptions to the pharmacy. Uses communication skills with appropriate medical terminology; and follows appropriate legal and ethical professional conduct; authorized individuals will use the ChenMed Rx system to support physician medication dispensing. Under the direction of the Center Clinical Manager oversees center quality activities including but not limited to: equipment checklists, monthly self assessments, waived testing quality control, inventory management Organizes and participates in quality improvement activities that promote center safety and improved patient outcomes.
