Position Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises the daily nursing activities of Certified Nursing Assistants. This role works in accordance with federal, state, and local regulations and supports the Director of Nursing to ensure the highest level of quality care is consistently delivered.


Shifts Available

Full-Time & Part-Time Positions

  • 1st Shift: 6:00 a.m. – 2:30 p.m.

  • 2nd Shift: 2:00 p.m. – 10:30 p.m.
    $0.75/hour shift differential

  • 3rd Shift: 10:00 p.m. – 6:30 a.m.
    $1.00/hour shift differential


Essential Duties & Responsibilities

  • Direct and supervise day-to-day activities of nursing assistants

  • Coordinate shift services, programs, and resident care activities

  • Provide leadership and guidance to assigned nursing staff

  • Admit, transfer, and discharge residents as required

  • Complete and maintain accurate medical records and documentation

  • Receive and transcribe physician orders

  • Prepare, administer, and document medications as ordered

  • Complete incident and accident reports per facility policy

  • Perform routine and skilled nursing procedures, including:

    • Catheter care

    • Tube feedings

    • Wound care and dressing changes

    • Suctioning

    • Colostomy and drainage care

    • Range of motion exercises

    • End-of-life care

  • Conduct rounds with physicians as needed

  • Ensure compliance with safety regulations and proper equipment use

  • Maintain resident privacy and confidentiality (HIPAA compliant)

  • Foster a calm, respectful, and supportive environment for residents and staff

  • Participate in shift reports and interdisciplinary communication

  • Perform other duties as assigned


Qualifications

  • Current Illinois LPN license in good standing

  • CPR Certification required

  • Long-term care experience preferred

  • Knowledge of nursing practices, regulations, and standards of care

  • Ability to read, write, speak, and understand English

  • Ability to make independent decisions in resident care situations


Physical & Mental Requirements

  • Ability to move intermittently throughout the workday

  • Ability to lift, push, pull, and/or carry at least 25 pounds up to 5 feet and move that weight up to 50 feet

  • Ability to manage the emotional and mental demands of the position

  • Ability to work independently with flexibility and professionalism

  • Strong interpersonal skills with residents, families, staff, and support agencies

How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation.
