Job Summary

Allure Healthcare Services is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. In this role, you will provide high-quality patient care, support resident well-being, and make a meaningful impact on the lives of those you serve.

Responsibilities

Provide direct patient care under the supervision of Registered Nurses and other healthcare professionals
Develop and implement personalized care plans for residents with varying healthcare needs
Administer medications, treatments, and therapies as prescribed by physicians
Monitor resident progress and report changes or concerns to Registered Nurses and physicians
Collaborate with interdisciplinary teams to ensure seamless care transitions and outcomes
Participate in ongoing education and training programs to maintain current knowledge and skills

Requirements

Current LPN licensure in the state of Illinois

How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Key Responsibilities Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. SUMMARY: In need of a LPN or a RN in which the individual would be responsible for providing direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing to ensure that the highest degree of quality care is maintained at all times. ESSENTIAL DUTIES AND RESPONSIBILITIES: Direct the day-to-day functions of the nursing assistants. Meet with your assigned nursing staff, as well as support personnel, in planning the shift’s services, programs and activities. Make written & oral reports/recommendations concerning the activities of the shift as required. Admit, transfer and discharge residents as required. Provide leadership to nursing personnel assigned to your unit/shift. Complete & file required recordkeeping forms/charts upon the resident’s admission, transfer and/or discharge. Receive & transcribe telephone orders from physicians & record on the Physician’s Order Form. Chart nurse’s notes in an informative & descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care.
