Aspire Senior Living of Warsaw is seeking a detail-oriented LTC MDS Coordinator (RN) to manage and ensure the accuracy and compliance of the Minimum Data Set (MDS) assessments for residents. The MDS Coordinator (RN) role involves coordinating assessments, care plans, and ensuring compliance with federal and state regulations in a long-term care (LTC) setting.

MDS Coordinator (RN) Key Responsibilities:

  • Complete and oversee MDS assessments and documentation

  • Ensure timely submissions to CMS
