
Hospice Nurse Practitioner
Certified Registered Nurse Practitioner (CRNP) – Hospice
Primarily In‑Office | Regional Team | No Nights or Weekends
Serenity Hospice is seeking a compassionate and experienced Certified Registered Nurse Practitioner (CRNP) to join our dedicated regional hospice team. This is a primarily in‑office position focused on meaningful, face‑to‑face clinical visits with hospice patients while collaborating closely with physicians and interdisciplinary care teams.
In this role, you will play a vital part in ensuring exceptional end‑of‑life care by conducting required hospice face‑to‑face evaluations and supporting high‑quality, compliant, and compassionate patient care across our service region.
Position Highlights

- No nights, weekends, or on‑call requirements

Key Responsibilities
- Conduct face‑to‑face hospice patient visits to evaluate continued eligibility in accordance with CMS Hospice Eligibility Guidelines
- Collaborate with the Hospice Medical Director and attending physicians to review patient status and adjust care plans as appropriate
- Provide clinical leadership and guidance to the interdisciplinary hospice team
- Support symptom management strategies and clinical decision‑making
- Authorize medications and treatments in accordance with state regulations and patient care needs
- Ensure accurate, timely, and compliant clinical documentation
- Participate in Interdisciplinary Group (IDG) meetings and care planning discussions
- Work primarily from an assigned Serenity Hospice office, with occasional local travel to patient homes or care facilities
Qualifications
- Current and active CRNP license in Pennsylvania
- Hospice or palliative care experience required
- Thorough understanding of CMS Hospice Eligibility Guidelines
- Strong interpersonal, organizational, and documentation skills
- Ability to work collaboratively within an interdisciplinary team
- Willingness to travel locally within the region, as needed

Locations & Regional Coverage
Serenity Hospice operates four Care Centers in Pennsylvania. Applicants must be able to work out of one of the following offices:
- Bensalem, PA
- Lancaster, PA
- Bethlehem, PA
- Wilkes‑Barre, PA
Ability to commute to the assigned office location is required.
