Duties and Responsibilities:

ENSURE THE SAFE DELIVERY OF OPTIMAL RESIDENT CARE SERVICES:

  • Accept assignment and make rounds at a minimum at the beginning of the shift, throughout the shift and at the end of the shift
  • Provide direct resident care according to the CNA daily plan of care
  • Take and record vital signs (temperature, pulse and respiration) and weights as directed
  • Assist with physical examinations, diagnostic tests and nursing procedures as needed
  • Report changes in the residents' condition and the need for modifications to the CNA Care plan
  • Complete assignment based on resident, unit needs
  • Escort resident to appointments, activities in or out of the facility as assigned
  • Provide post-mortem care as needed
  • Maintain awareness of the emergency equipment such as suctioning machine and oxygen
  • Assist medical and nursing staff during emergency situations

ENSURE RESIDENTS' SAFTEY IN ACCORDANCE WITH RESIDENT SAFETY PROGRAM:

  • Follow the safety features of the plan of care
  • Ensure that all residents have the appropriate ID wristbands on, as well as nametags on closets, beds, doors and wheelchairs. Report omissions to nurse
  • Check integrity of the sensormatic ID band on designated residents
  • Keep resident areas free of environmental hazards
  • Follow facility policy and procedures and the plan of care for the use of restraints
  • Explain use of call light bell and provide within resident's reach
  • Respond to call light bell promptly
  • Maintain safety during lifting, moving, positioning and transfer of residents as per facility protocol.
  • Report any equipment or furniture in need of repair
  • Report promptly any resident who is unaccounted for during meals or any other time during the shift
  • Utilize good judgment in the application of safety and protective devices
  • Perform Heimlich Maneuver when professional staff is not at the scene. Alert professional staff to continue intervention as needed
  • Ensure that all visitors to the unit and staff are properly identified
  • Otherwise adhere to the resident safety program while performing day to day responsibilities
  • Report observations that are inconsistent with resident safety program or violations of the policy to immediate supervisor and/or Chief Safety Officer


REQUIREMENTS:

  • Current New York State Nurse Aide Certification
  • Experience working in a health care setting
  • High School diploma or equivalent
  • Effective oral and written communication skills
  • Must be able to read, write and speak and understand the English language, knowledge of other language helpful
  • Must be able to demonstrate patience and compassion

This position will assist in patient care throughout the nursing areas and/or stay suites is responsible for the safe transport of patients via wheelchair or stretcher, dispatch services and appropriate documentation of services. Provides the services of an observer for any patient on a patient care unit who has been assessed to require constant observation to maximize his/her safety. Performs general receptionist and clerical duties to facilitate efficient functioning of the nursing unit and/or stay suites to post ASC patients.
