Job Responsibilities

• Relying on orders and care plans from a virtual care team to provide high-quality nursing care to patients during an acute phase of illness
• Working in conjunction with virtual nurses and hospitalists to identify any needs and interventions for patients in a hospital at home setting
• Participating in patient home visits and providing transition of care support
• Taking patients' blood pressure and other vital signs and monitoring their overall mental and physical condition
• Providing the virtual care team with timely and accurate patient reporting
• Documenting the health histories of patients and information about their symptoms
• Helping patients with activities of daily living (ADLs), such as eating, personal hygiene, and toileting
• Supporting a trusting patient-provider relationship by relaying patient concerns to the care team, demonstrating empathy, and providing companionship

Qualifications and Skills

• Active, unencumbered New Jersey CNA license
• Experience in an acute care, in-patient, hospital setting
• Current American Heart Association CPR for Healthcare Providers Certification
• Hepatitis B and MMR records, and current influenza vaccination
• Ability to lift a minimum of 50 lbs
