Responsibilities:

  • Provide skilled nursing care to patients in a skilled nursing facility setting, adhering to established protocols and guidelines

  • Conduct patient assessments, develop care plans, and implement interventions to meet individualized needs

  • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes

  • Perform various medical procedures, administer medications, and provide wound care as needed

  • Maintain accurate and detailed records of patient care, including progress notes and discharge summaries

  • Participate in quality improvement initiatives to enhance patient satisfaction and outcomes

Requirements:

  • Current LPN/RN license in the state of Ohio

  • Ability to work weekends, rotating shifts, and flexible scheduling to meet patient care needs

  • Strong communication and interpersonal skills to effectively collaborate with patients, families, and healthcare team members
