Key Responsibilities: • To perform or assist the resident with completing Activities of Daily Living (ADL). • Respond to residents call lights to provide maximum comfort, safety and privacy. • Transport residents by ambulation or by wheelchair to areas of community (dining room, lobby and activities). • Report any significant findings/observations to nurse immediately. • Assist residents on admission, transfer and discharge to make a smooth transition. • Responsible for the safekeeping of residents’ personal possessions. • Keep resident rooms neat and safe. • Follow proper infection control techniques • Respect the rights of each resident, particularly privacy and confidentiality. • Re-teach resident to perform ADL skills. • Provide restorative care to prevent pressure sores, contractures, and complications. • Participate in Continuous Quality Improvement and ensures follow-up occurs. • Converse with resident and family; assist them on adjustment to the community. • Ensure adequate hydration and nutrition of resident; report deviations from norm to the Charge Nurse promptly. • Perform blood pressure, pulse, respirations, intake/output and weights accurately. Qualifications: • Certified and state tested as a Certified Nursing Assistant in the state employed; certificate must be active, valid, and in good standing. The Patient Care Technician , working under the direct supervision of Registered Nurse, is primarily responsible for providing direct, non-invasive bedside care to patients. Essential Functions: Prioritizes and delivers basic patient care, including bathing, toileting, feeding, grooming, etc. Performs and documents vital signs, measures and records intake and output, weights, specimen collections, etc. Transfers patients utilizing proper body mechanics and safe patient handling techniques. Reports any changes observed in condition or behavior of patient to appropriate nursing personnel. Assists in preparing unit, patient rooms, and patient beds for receiving patient admissions. Maintains bed and room in a clean, neat, safe, and comfortable manner. Removes supplies and equipment not in use; placing furnishings and equipment out of the path in order to provide a safe, neat, and organized area. Adheres to patient rounding expectations and standards. Assists in maintaining a physical, social, and psychological environment conducive to providing maximal patient comfort and optimizing healing.
