Responsibilities

Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
Completing history and physical examinations by evaluating the patient's medical condition and health history
Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
Completing and maintaining accurate clinical notes
Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
Ensuring compliance with all state and federal legal and regulatory requirements
Additional responsibilities as assigned

Qualifications

Graduate from an accredited School of Nursing and accredited Nurse Practitioner program
Current DEA license, if applicable to state regulations
Current nursing license in state of practice
Current CPR certification
Master’s degree with minimum of one year NP experience
One year hospice or palliative care experience preferred
Accreditation from an approved certifying body for advanced practice nursing as required by state of practice
Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics)
Commitment to clinical and documentation excellence

Within the Stead Family Department of Pediatrics, this position works in collaboration with the attending physicians and will work within their scope of practice to provide excellent clinical care for patients with cancer and recipients of Bone Marrow Transplantation and cellular therapies within the Division Hematology, Oncology and Bone Marrow Transplantation/Oncology. In accordance with scope of practice as outlined by applicable state law, regulations, institutional policies and practice agreements, the position will conduct comprehensive health history and physical assessments, establish medical diagnoses, order and evaluates diagnostic studies, and develop treatment plans. Position Responsibilities Functions independently and/or as a member of an interdisciplinary team to evaluate, plan and deliver care to patients. Obtains and documents a health history; performs and documents complete, system-focused, or symptom-specific physical examination, assessment, and plan of care. In collaboration, provides healthcare services for primary, acute, and complex care of patients and manages patient's overall care, initiates appropriate referrals and consultations, and identifying expected outcomes for diagnoses. Orders, performs, interprets, and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Forms differential diagnoses and treats acute and chronic oncological conditions. Prescribes therapeutic interventions both pharmacologic, non-pharmacologic and surgical/procedural, needed to achieve expected outcomes. Required to become competent in ordering chemotherapy, immunotherapy, and targeted therapy including the management of side effects. Opportunity to complete a specific clinical training in lumbar punctures and bone marrow aspirations. After completion, additional privileges would be obtained.
