In addition, you utilize age-appropriate clinical assessment and intervention with patients to support their psycho-social and medical needs of patients and families. The care of patients does not stop in the office: as an RN Ambulatory Care Manager you will connect with eligible patients for initial and periodic telephonic outreach calls focusing on clinical assessments, early symptom recognition and intervention, self-management abilities, and health literacy.

Experience
