Rolling Hills Healthcare and Rehabilitation Center is currently seeking an outstanding Full Time MDS Coordinator to join our team of professionals. We're looking for a motivated, team-oriented MDS Coordinator. The right candidate must have good communication skills in order to work effectively with the Interdiciplinary Team and keep acurate, detailed records.

Familiarity with Point Click Care (PCC) and 1-2 years of recent MDS experience preferred.

Current NJ RN license in good standing required.

· Responsible for scheduling and maintaining accurate timely completion of MDS using PCC Software system.

· Ensure all MDS are in compliance with State and Federal regulatory requirements

· Ensure timely and accurate electronic transmission to the state

· Attend and manage weekly utilization review meetings and maintain open communication with therapy and team members

· Participate in developing/updating/reviewing/completion of individualized Care Plans

· Monitor facility staff in regards to MDS process, as needed

· Monitor facility QM reports to ensure proper corrective action can be implemented when potential problems arise

· Participates in facility meetings as requested and completes all other duties as assigned

· Responsible for inputting and updating appropriate ICD-10 codes for all residents

Range of pay 90,000-105,000 based on experience.

Job Description PER DIEM Essential Job Functions: Provides total, comprehensive care to patients by applying the perioperative nursing process in an organized and systematic manner according to institutional policy in keeping with patient safety principles. Collaborates with the patient, significant others and ancillary departments to become a multidisciplinary health care team whose goal is to provide care that is continuous and well-coordinated Communicates proactively to the health care team plan of care changes in patient condition, the patient progress, the discharge plan and other relevant information. Demonstrates knowledge of nursing responsibilities regarding anesthesia administration. Demonstrates the ability and competency to respond quickly and efficiently in emergency situations. Implements nursing action in transferring the patient according to prescribed plan. Creates and maintains a sterile field Performs surgical counts according to facility policy and procedure. Dispenses drugs and solutions according to policy Competently manages the care of the surgical patient. Provides equipment and supplies based on patient needs Must have excellent communication and computer skills Qualifications Education: Associates degree required. Bachelor’s degree preferred. Required License: Valid New Jersey State Nursing License Certifications: BLS Required Relevant Experience: At least 1 year of perioperative experience as a Registered Nurse Scrubbing experience preferred PER DIEM Rate: $56.50 Pay Range: $45.14 - $56.44 Hourly The provided compensation range is based on industry standards and salary determinations will be made based on numerous factors including but not limited to years of experience and location of position.
