The CHHA will provide routine individualized healthcare based on a care plan set by LVN to the elderly, convalescents, or persons with disabilities at the patient's home or in a care facility. Monitor or report changes in health status. May also provide personal care such as bathing, dressing, and grooming of patient. 

ESSENTIAL DUTIES AND RESPONSIBILITIES

The essential functions include, but are not limited to the following:

  • Provide companionship and socialization
  • Escort clients to recreational activities and medical appointments
  • Perform light housekeeping duties, as needed, to maintain a safe environment (i.e. changing bed linens, laundry, dishes, etc.)
  • Help clients with personal care needs (i.e. Home duties, cooking, light cleaning, medication reminders, etc.)
  • Assist clients with Activities of Daily Living (ADLs)
  • Perform light housekeeping duties, as needed, to maintain a safe environment (i.e. changing bed linens, laundry, dishes, etc.)
  • Teach and encourage self-care skills
  • Maintain complete and accurate documentation of clients’ care
  • Maintain confidentiality in all matters related to the client's care
  • Perform other duties as assigned

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)

  • One year of Caregiving experience (preferred)
  • CHHA (preferred) or CNA (required)
  • Driver’s License, Car Insurance, Car Registration
  • Strong clinical assessment, organizational, and communication skills
  • Ability to handle multiple assignments simultaneously
  • Excellent oral and written communication skills
  • Excellent customer relationship skills
