Position Summary


Saturday & Sunday 6am-10pm

The LPN/LVN provides direct nursing care to residents within the scope of licensure, following established nursing standards, facility policies, and physician orders. This role plays a vital part in promoting comfort, dignity, safety, and quality of life for our residents while working collaboratively with the interdisciplinary care team.

Key Responsibilities

  • Provide compassionate, resident-centered nursing care in accordance with state licensure, facility policies, and physician orders.

  • Administer medications safely and accurately, following the 10 Rights of Medication Administration and scope of practice.

  • Perform nursing treatments, procedures, and daily resident care as prescribed.

  • Monitor resident conditions and promptly communicate changes, symptoms, or reactions to the MD/NP and nursing leadership.

  • Assist physicians and nurse practitioners with examinations, treatments, and special procedures requiring technical and manual skills.

  • Participate in the development, implementation, and updating of individualized care plans under the supervision of the MD/NP and RN.

  • Support rehabilitation goals by assisting with treatments and encouraging resident independence when appropriate.

  • Provide appropriate pain management interventions and educate residents and families on pain management strategies.

  • Treat all residents and families with respect, dignity, and compassion, honoring their psychosocial, cultural, ethnic, and spiritual needs.

  • Actively involve residents and families in care planning and decision-making.

  • Assist with resident admissions, discharges, and transfers to ensure smooth transitions of care.

  • Identify psychosocial and discharge planning needs and collaborate with Social Services and the Discharge Planner for both in-facility and post-discharge support.

  • Maintain professional communication with residents, families, and the interdisciplinary care team.

Qualifications

  • Current and valid LPN/LVN license in the state of Texas

  • Current CPR Certification (BLS preferred)

  • Strong clinical skills and attention to detail

  • Compassionate, patient-focused approach to care

  • Ability to work collaboratively within an interdisciplinary team

  • Excellent communication and documentation skills
