Overview
Functions as a clinical liaison throughout the continuum of patient care and resource. Facilitates a team approach that includes surgeons, physicians, pre admitting, surgical and floor nurses, pharmacists, physical therapists, dietitians or behavioral health. In the oncology service area, ensures proper education of staff and patients, also family members and community, when appropriate.
Responsibilities
Principal Duties and Responsibilities (*Essential Functions)
*Identifies patient needs through consultation with physicians, review of records and interviews with the patient and family.
*Works with all services to provide consistent, organized and understandable care for the diagnostic and treatment phases of a patient’s treatment plan.
*Provide care coordination by scheduling and coordinating diagnostic and treatment planning with the primary care physician, consulting physicians, and hospital departments in order to facilitate the patient’s experience and increase the efficiency and effectiveness of the patient’s recovery.
*Performs patient assessments, interviews patients and families, and makes appropriate referrals.
*Provides education to the oncology patients and family members regarding specific health issues that may arise during the treatment and recovery stages.
*Acts a patient advocate by voicing needs and concerns throughout the healthcare system.
*Connects patients and families to necessary support services and community resources as needed.
*Participates in conferences, accreditation meetings by collecting data and providing reports.
*Collaborates with all members of the health care team in the management of the plan of care for the patient receiving treatments.
*Assist physician and staff with exams and other procedures as needed.
Performs additional duties specific to the oncology service area.
Qualifications
Job Specific (Minimum Requirements)
Knowledge, Skills, and Abilities
Knowledge of national treatment protocols, guidelines and standards of care for the management of oncology patients.
Demonstrated clinical competency in oncology patient care delivery, teaching, and management of patients and families.
Excellent oral and written communication skills.
Ability to work independently, with minimal direct supervision, and as a member of multidisciplinary team.
Ability to provide compassionate, kind, and caring approach in patient interactions.
Education
Bachelors Degree in Nursing is Required
Masters Degree in Nursing is Preferred
Experience
5 years of experience as a Clinical Registered Nurse are Required
1 year of Teaching and managing patients in oncology is Preferred
Certifications and Licensures
Licensed Registered Nurse is Required
CPR is Required
