The Wound Care Nurse is responsible for assessing, treating, and monitoring residents with acute, chronic, and post-surgical wounds in a long-term care setting. This role ensures evidence-based wound management, regulatory compliance, accurate documentation, and staff education to promote optimal healing and prevent complications.
Perform comprehensive wound assessments upon admission, weekly, and as condition changes
Identify, stage, and measure pressure injuries, surgical wounds, diabetic ulcers, venous/arterial ulcers, and skin tears
Develop and implement individualized wound care treatment plans
Perform wound treatments and dressing changes per physician orders and facility protocols
Monitor wound healing progress and revise care plans as needed
Communicate wound status changes to physicians, nurse practitioners, and interdisciplinary team members
Obtain and implement physician orders for wound treatments, labs, cultures, and referrals
Ensure timely and accurate wound documentation in the medical record
Photograph wounds per facility policy and regulatory requirements
Participate in care plan meetings and interdisciplinary rounds
Educate nursing staff and CNAs on wound prevention, skin care, offloading, and pressure injury prevention
Assist with pressure injury prevention programs, including risk assessments and turning schedules
Ensure compliance with state, federal, and CMS regulations related to wound care
Assist with survey readiness and respond to wound-related deficiencies if applicable
Monitor wound care supplies and recommend appropriate products
Collaborate with dietitians regarding nutrition and wound healing needs
Participate in quality assurance and performance improvement (QAPI) initiatives
Qualifications
Current, unrestricted RN or LPN/LVN license (state-specific)
Minimum 1–2 years of nursing experience, preferably in long-term care, skilled nursing, or rehabilitation
Wound care experience strongly preferred
