This role assists the DHW with administrative duties of the wellness department. Additional duties may include; ordering supplies, med cart audits, tracking lab orders, assisting with the preparation of required document prior to in house physician visits, assisting with resident file organization, Memory Care Safety Room checks, scheduling and finding replacements for call offs.

 The Resident Care Coordinator is responsible for the coordination of resident care needs including, but not limited to drafting the schedule, maintenance of required logs such as the physician appointment log and the pharmacy log, filing and organization of the clinical record, retrieval of signed physician orders, etc. Additionally, the Resident Care Coordinator assists the Director of Health & Wellness, as needed, to train new Care Managers, and Medication Technicians. In the absence of the Director of Health & Wellness, assumes the responsibility of supervision and delivery of resident care services in compliance with Terra Bella expectations, policy and procedure and State regulations.

Responsibilities:

  • Ensures resident care staffing is within regulatory and company requirements with a 4-week rolling schedule submitted to the Director of Health & Wellness for approval at least two weeks in advance of posting due date.
  • Maintains the resident care scheduling book with current approved requests for time off noted on the scheduling calendar for planning purposes.
  • Performs daily filing of all documents relevant to the resident record in chronological order. Performs a quality assurance review of the documents prior to filing to ensure items are dated, verified and signed by the physician if appropriate.
  • Performs the ongoing task of verbal order signature retrieval, ensuring all orders are returned signed by the physician with 14 days of the date the order is received.
  • May conduct weekly cart audits of each medication cart following the cart audit process and submits the audit sheet to the Director of Health & Wellness and Executive Director for review in the event that a Support Nurse cannot complete the audit.
  • Assists with data collection, reporting processes as requested.
  • Performs a daily review of medication administration process ensuring medications are passed timely and accurately. Intervenes as needed to provide support/ training during the med pass.
  • Provides thorough and complete new hire training for new care staff team members ensuring each new team member is in full compliance with all policies, procedures and training relevant to their individual roles before completion of orientation.
  • Takes physician's orders following the medication order process and maintains adequate records of order transcription and verification.
  • Ensures that all Residents Rights are protected.
  • Notifies the Director of Health & Wellness of changes in resident's condition, and any unusual or abnormal observations. (In the absence of the Director of Health & Wellness, notifies the physician directly) Ensures appropriate documentation is entered into the resident’s record and the 24-hour shift report relevant to changes in condition and physician communication.
  • Schedules appointments, and arranges for transportation of residents for diagnostic, therapeutic, dental or medical services. Ensures compliance with the physician visit log process/procedure.
  • Understands and implements the Fire Safety and Disaster Plan and evacuation procedures of the community during drills and actual emergencies.
  • Performs initial review of the monthly Medication Administration Record verification to ensure orders are compliant.
  • Assists the Director of Health & Wellness to ensure compliance with all required logs, policies and procedures relevant to medication administration.
  • Performs building rounds at the beginning and end of each shift, evaluating the care provided and ensuring resident needs are met.
  • May be required to work overtime in an emergency situation.
  • Sets a good example for residents and other staff members by appearance, conduct and interest in job.
  • Participates in Manager on Duty Program.
  • Performs all other duties as assigned by the Director of Health & Wellness.

Lead

  • Scheduling and documenting team member call off records.
  • Rotating on-call with Director of Health and Wellness.
  • Assists with ordering supplies.
  • Assists with Med Cart audits.
  • Tracks and monitors lab orders.
  • Assists with the preparation of required document prior to in house physician visits.
  • Conducts Memory Care Room safety checks.
  • MAR monthly reconciliations.
  • Entering Lab orders for draw days.
  • Coordinating training and orientation days for new team members.
  • Thinning charts and filing medical records per regulatory guidelines.
  • Assisting the Director of Health and Wellness with care manager and medication technician evaluations.

 Qualifications:

  • Certified Nurse’s Aid certification/Qualified Medication Aid certification
  • High School diploma/GED
  • Must be 18 years of age.
  • CPR Certification
  • First Aid Certification
  • Previous experience working with seniors preferred

The MDS / Care Plan Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses and determines the health status and level of care of all new admissions. Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change. Communicates level of care for new resident to all disciplines. Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state and medical standards. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. Verifies electronic submissions of MDS, performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. Assists disciplines in formulating and revising care plans. Ensures that resident’s present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. Evaluates resident care plans for comprehensiveness and individuality. Assesses the achievement or lack of achievement of desired outcomes. Ensures that resident’s care plan is reassessed and revised appropriately. Responsible for all level of care changes within the facility. Notifies all departments when a level of care change has been made. Generates appropriate forms to complete level of acuity and changes. Transmits forms to the appropriate agency for processing as required by state law. Other duties as assigned. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
