Job Details:

Scheduled hours: Day - Varies | Occasional Weekend Requirements

Provides basic patient care under the direction of a licensed nurse. Serves as patient’s ‘Personal Care Champion’ by assisting them with activities of daily living. Provides care & support in the home setting or hospice house with special attention to patient personal care needs in end of life care. Perform visits and/or shift work. Works independently providing personal cares to individuals in their homes and facilities. Must be able to work independently, have good time management skills, timely documentation, and a valid drivers license.

Prefer Iowa Certified Nurses Aide

Maintains valid licensure/certification/registration if required

Current Drivers license and proof of insurance for staff performing visits.
