Responsibilities:

  • Provide high-quality, patient-centered care to our residents, including wound care, medication management, and education

  • Collaborate with interdisciplinary teams to develop and implement resident care plans

  • Monitor and report changes in resident conditions, including vital signs and lab results

  • Administer medications and treatments as ordered by physicians

  • Ensure accurate and timely documentation of resident care and services

  • Participate in quality improvement initiatives to enhance resident outcomes and satisfaction

Requirements:

  • Current RN or LVN licensure in the state of Texas

  • Minimum 0 years of experience (new graduates welcome to apply!)

Patient Care Technician PCT/CNA - Full Time Day Shift

The Patient Care Technician , working under the direct supervision of Registered Nurse, is primarily responsible for providing direct, non-invasive bedside care to patients. Shift : 12-hour shifts Essential Functions: Prioritizes and delivers basic patient care, including bathing, toileting, feeding, grooming, etc. Performs and documents vital signs, measures and records intake and output, weights, specimen collections, etc. Transfers patients utilizing proper body mechanics and safe patient handling techniques. Reports any changes observed in condition or behavior of patient to appropriate nursing personnel. Assists in preparing unit, patient rooms, and patient beds for receiving patient admissions. Maintains bed and room in a clean, neat, safe, and comfortable manner. Removes supplies and equipment not in use; placing furnishings and equipment out of the path in order to provide a safe, neat, and organized area. Adheres to patient rounding expectations and standards. Assists in maintaining a physical, social, and psychological environment conducive to providing maximal patient comfort and optimizing healing. Promptly and appropriately responds to patient and family calls and requests. Appropriately designates applicable charges to individual patients using hospital’s established system. May be required to work during inclement weather and other staffing emergencies. Provides an environment conducive to safety for patients, visitors, and staff. Assesses environmental risks for safety and implements appropriate precautions. Complies with appropriate and approved safety and Infection Prevention standards. Physical Requirements Over the Course of a Shift: A significant amount of standing, walking, bending, reaching, carrying, lifting, pushing, and pulling, often for prolonged periods of time. Both gross and precise motor functions. Lifting/exerting of up to 50 lbs. Possible exposure to bodily fluids. Visual acuity required for patient assessment and documentation of care.
