Job Summary:
The RN nurse navigator serves as a critical resource for the inpatient unit as well as the Strong Futures Program. As a key member of the treatment team, they are an access point for adolescents presenting for inpatient acute services and discharged patients that require follow-up services. The Nurse Navigator will serve as a community liaison, communicating bed availability and assisting referral sources with admission presentations. The RN will have a working knowledge of admission criteria, registration, and the Strong Futures referral process. They will be able to work independently and have strong communication skills. The nurse navigator will reach out to patients and families providing education an ensuring they have the information needed to successfully transition from inpatient care to outpatient.
Essential Duties and Responsibilities
• Supports and promotes the mission and values of Covenant Health Ministry.
• Liaison to the public providing education and referral development.
• Support and educate individuals, families, and other callers around the admission procedure, and what to expect on the inpatient unit.
• Communicate daily and often with community partners and outside facilities for potential direct admissions.
• Collect patient’s medical and behavioral history on a screening tool for presentation to the provider for review. Perform Medication Verification.
• Once a patient is accepted for admission reaches out to the parent or guardian seeking consent for treatment and answering any questions they may have regarding the inpatient program.
• Create a new admission chart and assign a bed on the unit in EPIC. Coordinate with registration staff around consent and workflow.
• Coordinate patient transport with the inpatient unit staff and receiving nurse, nursing
supervisors, and security if needed.
• If needed, helps complete the nursing admission assessment, safety screens, belongings record, and coordinate skin assessment with receiving nurse.
• May assist as needed on the behavioral units: cover breaks, assist with CVO assignments, respond to Code Gray Emergencies when needed for surges and staff support, and any other duties as assigned.
• Ensure that patients referred to the Strong Futures Program have an appointment, preferably prior to discharge. Reaches out to legal guardian to schedule the appointment and review the visit requirements.
• Works with the clinician and social worker to ensure scheduling availability.
• Registers the appointment.
• If needed follows up with families after a missed appointment.
• Will screen incoming calls and act as a resource for patients and family members, communicating patient needs directly to the care team members.
• The nurse navigator will provide patient and family education groups.
• Works closely with the program management to ensure an efficient, and effective aftercare service is provided.
• Maintains all required competencies and completes annual compliance courses on time.
• Supporting student learning is a shared responsibility. Every team member is expected to
contribute to creating a welcoming environment and to actively assist students in their
educational experience, regardless of their specific role.

- Crisis Prevention Institute (CPI) training preferred or obtained before completion of
- orientation.
Education and Experience
- Current Maine RN license required.
- Basic Life Support certification required.
- Graduate of an approved nursing program required.
- Certified as being trained in the Crisis Prevention Institute (CPI).

Comp Range:
$31.84 - $43.85Rate of pay displayed reflects the beginning of the pay scale. At the time of an offer, determination of your offer will reflect your skills and experience as it relates to the position.
